摘要
目的研究不同肌松药对乳腺癌改良根治术患者的麻醉效果,并分析诱导期应激反应变化。方法选择在我院进行乳腺癌改良根治术治疗的乳腺癌患者94例,随机分为观察组和对照组各47例。观察组采用罗库溴铵进行麻醉诱导,对照组采用顺式阿曲库铵麻醉诱导。对比两组麻醉效果、不同时间点肾上腺素(E)、去甲肾上腺素(NE)以及平均动脉压(MAP)、心率(HR)、血氧饱和度(Sp O2)等变化,同时记录术后不良反应。结果观察组起效时间显著短于对照组,而麻药维持时间以及气管拔管时间显著长于对照组(P<0.05);两组在T0时E、NE对比无显著差异(P>0.05),但T1和T2时观察组E、NE均显著低于对照组(P<0.05);两组在T0时MAP、HR、Sp O2对比无显著差异(P>0.05),但T1和T2时观察组MAP、HR均显著低于对照组(P<0.05),而Sp O2显著高于对照组(P<0.05);观察组不良反应发生率为12.77%,显著低于对照组的31.91%(P<0.05)。结论罗库溴铵用于乳腺癌改良根治术,具有更好的麻醉效果,对应激反应、血流动力学的影响小于顺式阿曲库铵,且起效快,术后不良反应少,具有更高的应用价值。
Objective To stu dy the anesthetic effects of different muscle relaxant on modified radical mastectomy for breast cancer and analyze the changes in the stress during the induction period. Methods A total of 94 patients with breast cancer treated in our hospital with modified radical mastectomy for breast cancer were selected and randomly divided into an observation group and a control group(n=47, respectively). Rocuronium was applied to the observation group for anesthesia induction and cisatracurium to the control group. The changes in the anesthetic effects, adrenalin(E) and methylepinephrine(NE) at different time points, MAP, HR and Sp O2 in the two groups were compared and adverse reactions were recorded. Results The onset time in the observation group was significantly shorter than that in the control group, and the anesthetic duration and extubation time were much longer than those in the control group(P〈0.05). There was no significant difference in E and NE between the two groups at T0(P〈0.05), but the E and NE in the observation group at T1 and T2were significantly lower than those in the control group at the same time points(P〈0.05); there was no significant difference in MAP, HR and Sp O2 between the two groups at T0(P〉0.05), but the MAP and HR in the observation group at T1 and T2were significantly lower than those in the control group and Sp O2 was significantly higher than that in the control group(P〈0.05). The incidence of adverse reactions in the observation group was 12.77%, which was significantly lower than that in the control group(31.91%)(P〈0.05). Conclusion The application of rocuronium in modified radical mastectomy for breast cancer has better anesthetic effects and minor impacts on the stress and hemodynamic effects than that of cisatracurium; it has fast effects, causes less postoperative adverse reactions, and has higher value in clinical application.
