高压氧治疗糖尿病足的疗效评价及对患者血清VEGF、bFGF水平的影响 被引量:39

摘要 目的评估高压氧治疗糖尿病足的临床疗效及其对患者血清血管内皮细胞生长因子(VEGF)、碱性纤维母细胞生长因子(b FGF)水平的影响。方法选取130例糖尿病足患者,按照区组随机化分为高压氧组和对照组。对照组给予糖尿病饮食、降血糖、局部创面处理、抗感染等常规治疗;高压氧组在常规治疗基础上予以高压氧治疗,高压氧治疗压力为0.20~0.25 MPa,患者吸纯氧80 min,1次/d,每周5 d,共8周。观察治疗前后空腹血糖、血液流变学的变化,并在治疗前后抽取患者血液,检测血清VEGF、b FGF水平的变化。结果治疗前后,与对照组比较,高压氧组治愈率(64.6%)和有效率(95.4%)更高,血液流变学得到改善;高压氧治疗后患者血清VEGF、b FGF升高,差异有统计学意义(P〈0.05),并且与对照组比较,高压氧组血清VEGF、b FGF升高更明显。结论辅助高压氧治疗糖尿病足疗效优于常规治疗,并且可降低患者血清VEGF、b FGF水平。 Objective To evaluate the clinical efficacy of hyperbaric oxygen in treatment of diabetic foot and its influence on patients' serum levels of vascular endothelial growth factor(VEGF) and basic fibroblast growth factor(b FGF). Methods Totally 130 patients with diabetic foot were chosen and divided into hyperbaric oxygen group and control group according to block random tumble. The control group was given conventional therapy such as diabetes diet, lowering blood sugar, local wound treatment and anti-infection, while the hyperbaric oxygen group received additional hyperbaric oxygen therapy. Hyperbaric oxygen pressure was 0.20-0.25 MPa. The patients inhaled pure oxygen for 80 min, once a day, 5 days a week for 8 weeks. The changes of fasting blood sugar and blood rheology were observed, and the changes of serum levels of VEGF and b FGF were detected before and after treatment. Results Compared with the control group, the hyperbaric oxygen group had higher cure rate and effective rate(64.6% and 95.4% respectively, P〈0.05), and better improvement of the blood rheology(P〈0.05) after treatment. In the hyperbaric oxygen group, serum VEGF and b FGF levels were significantly elevated after treatment(P〈0.05), which were obviously higher than those of the control group. Conclusions The hyperbaric oxygen treatment is superior to conventional treatment for diabetic foot, and can reduce serum VEGF and b FGF levels of the patients.
