摘要
目的：探讨瑞舒伐他汀治疗对急性冠脉综合征（ACS）患者颈动脉斑块以及血清中炎性因子水平的影响。方法：选取在我院心内科治疗的ACS患者82例，随机分成对照组与观察组各41例，对照组在入院以后予以ACS心内科常规治疗，包括采用阿司匹林、β-受体阻滞剂、血管紧张素转换酶抑制剂（ACEI）等药物治疗，不服用任何他汀类药物，观察组在心内科常规治疗基础上予以口服瑞舒伐他汀6个月治疗。观察两组治疗前后颈动脉斑块以及血清中炎性因子水平变化。结果：观察组治疗6个月后，患者颈动脉斑块的数量、面积、厚度以及IMT值较治疗前均有明显的缩小（P〈0．05），而对照组治疗前后患者颈动脉斑块的数量、面积、厚度以及IMT值均无明显的变化（P〉0．05）；观察组与对照组治疗后患者血清中TNF-α、IL-6以及MMP-9水平较治疗前均有明显降低（P〈0．05），但观察组各炎性因子水平改善较对照组更加显著（P〈0．05）。结论：瑞舒伐他汀治疗ACS，能显著发挥抗炎效应，明显改善患者颈动脉斑块的炎性反应及粥样斑块。
OBJECTIVE ： To explore rosuvastatin on inflammatory cytokines and in the treatment of carotid plaques in the patients with ACS. METHODS： 82 patients with ACS in our hospital were randomly divided into obsevation group （n = 41 ） and control group （n =41）. Control group received aspirin, beta blockers, angiotensin converting enzyme inhibitor （ACEI） and other drugs treat- ment. Observation group oraled rosuvastatin for 6 months on the basis of the treatment above. Carotid plaques and inflammtory cyto- kines levels before and after treatment in the two groups were observed. RESULTS ： After 6 months of treatment, the carotid plaques, area, thickness and IMT values were reduced more significantly in the observation group than those before treatment （ P 〈 0.05）. But there were no significant chang in the indexas above in the control group before and after treatment （ P 〉 0.05 ）. TNF - α, IL - 6 and MMP -9 levels reduced obviouesly after treatment than those after treatment in the two groups （P 〈 0. 01 or P 〈 0. 05 ） ; but inflammatory factors levels in the observation group were improved more obviously than those in the control group （ P 〈 0.05）. CONCLUSION ： In the treatment of ACS, rosuvcstatin may not only significantly exert anti - inflammatory effect, but also obviously improve the carotid plaques in the patients with inflammatory reaction and atheromatous plaque.
