摘要
目的探讨早期肠内营养护理改善重症胰腺炎感染后营养状态的临床效果,为临床护理提供理论指导。方法选取2013年2月-2015年2月在医院接受治疗的重症胰腺炎合并感染患者90例为研究对象,将其随机分为试验组和对照组,每组患者各45例,对照组患者采用常规护理方法+静脉配置肠外营养支持,试验组患者在常规护理的基础上采用早期肠内营养护理,比较两组患者各项生化指标(营养状态)、住院时间及并发症、病死率。结果两组患者治疗后血糖、血淀粉酶及C-反应蛋白等指标值均明显优于治疗前,差异有统计学意义(P<0.05),而治疗后试验组其生化指标改善情况与对照组比较,差异无统计学意义;试验组平均住院时间为(22.5±3.2)d,明显短于对照组的(31.4±4.5)d;试验组并发症发生率为11.1%,病死率为4.4%,对照组并发症发生率为48.9%,病死率为13.3%,两组比较差异有统计学意义(P<0.05);患者出院时其营养状态明显优于入院时(P<0.05),且试验组患者改善程度明显优于对照组(P<0.05)。结论早期肠内营养护理对改善重症胰腺炎合并感染患者临床效果显著,可有效改善患者营养状态,降低患者并发症发生率及病死率,值得在临床推广应用。
OBJECTIVE To explore the clinical effect of early enteral nutrition care on improvement of nutritional status of severe pancreatitis patients with infections so as to provide theoretical basis for clinical nursing.METHODS A total of 90 severe pancreatitis patients complicated with infections who were treated in the hospital from Feb 2013 to Feb 2015 were recruited as the study objects and randomly divided into the experimental group and the control group,with 45 cases in each group.The control group was treated with conventional nursing plus intravenous configuration of parenteral nutrition support,while the experimental group was given early enteral nutrition care based on the conventional nursing.The biochemical indicators(nutritional status),length of hospital stay,complications,and mortality rate were observed and compared between the two groups of patients.RESULTS The levels of blood glucose,blood amylase,and C-reactive protein of the two groups of patients were significantly higher after the treatment than before the treatment(P〈0.05);however,there was no significant difference in the improvement of the biochemical indicators between the experimental group and the control group after the treatment.The average length of hospital stay of the experimental group was(22.5±3.2)days,significantly shorter than(31.4±4.5)days of the control group.The incidence of complications was 11.1% in the experimental group,48.9%in the control group;the mortality rate was 4.4%in the experimental group,13.3%in the control group,and there was significant difference between the two groups(P〈0.05).The nutritional status of the patients was significantly better at the discharge from hospital than at the admission to hospital(P〈0.05),and the degree of improvement of the experimental group was remarkably better than that of the control group(P〈0.05).CONCLUSIONThe early enteral nutrition care can achieve remarkable clinical effect on improvement of nutritional status of the severe pancreatitis patients complicated with infections and reduce the incidence of complications and mortality rate,and it is worthy to be promoted in the hospital.
