摘要
目的：分析比较欣普贝生与宫颈扩张双球囊对足月妊娠孕妇产程及新生儿预后的影响。方法：选取2014年6月至2015年6月我院收治的112例足月妊娠初产妇为研究对象,随机分为欣普贝生组56例与宫颈扩张双球囊组56例。记录并比较两组孕妇促宫颈成熟效果、产程时间、妊娠结局及新生儿预后情况。结果：两组孕妇在应用促宫颈成熟措施后欣普贝生组宫颈Bishop评分显著高于双球囊组（t=4.447,P〈0.01）。欣普贝生组促宫颈成熟的总有效率为98.2%,显著高于双球囊组的85.7%（χ^2=4.35,P〈0.05）。欣普贝生组产妇活跃期与第二产程时间显著短于双球囊组（t=5.630、7.462,P〈0.01）;而急产人数显著多于双球囊组（χ^2=5.14,P〈0.05）。欣普贝生组孕妇出现强直性/过频宫缩例数及产后出血例数均显著多于双球囊组（χ^2=4.73、4.80,P〈0.05）。胎儿窘迫例数、新生儿Apgar评分及脐动脉p H值两组间比较差异均无统计学意义（P〉0.05）。结论：对于足月妊娠的产妇,欣普贝生促宫颈成熟效果较为明显,但会出现第二产程过短、急产、强直性宫缩及产后出血等不良反应,宫颈扩张双球囊促宫颈成熟效果较为温和安全。
Objective： To analyse and compare the effect of dinoprostone suppositories and double balloon cathete on labor and neonatal outcomes of term pregnancy maternal. Methods： 112 term pregnancy primipara from June2014 to June 2015 in our hospital were divided into dinoprostone suppositories group（ 56 cases） and double balloon group（ 56 cases） randomly. Cervical ripening effect,labor time,pregnancy outcomes and neonatal outcomes were recorded and compared between two goups. Results： Bishop score of dinoprostone suppositories group after treatment was higher than that of double balloon group（ t = 4. 447,P〈0. 01）. Total efficiency of cervical ripening of dinoprostone suppositories group（ 100. 0%） was better than that of double balloon group（ 91. 1%）（ χ^2= 5. 32,P〈0. 05）. Active phase and second stage of labor of dinoprostone suppositories group were shorter than those of double balloon group（ t = 5. 630,7. 462,P〈0. 01）. And the number of acute production of dinoprostone suppositories group was more than that of double balloon group（ χ^2= 5. 14,P〈0. 05）. The cases of ankylosing / too frequent contractions and postpartum hemorrhage were more than double balloon group（ χ^2= 4. 73,4. 80,P〈0. 05）. The cases of fetal distress,Apgar score and umbilical artery p H value between two goups had no significant differences（ P〉0. 05）. Conclusions： For full-term pregnancy maternity,dinoprostone suppositories has more obvious cervical ripening effect. But it is associated with adverse reactions like too fast second stage,radical production,tonic contractions and postpartum hemorrhage. Double balloon dilatation has more moderate and safe cervical ripening effect.
