细胞因子与狼疮合并妊娠的临床特征的相关性分析 被引量:6

摘要 目的探讨多种细胞因子在系统性红斑狼疮(Systemic lupus erythematosus,SLE)合并妊娠中的相互影响,为细胞因子作为SLE合并妊娠的病情活动程度及预后判断的监测指标提供依据。方法取SLE妊娠组30例及健康妊娠组30例,妊娠早期、中期、晚期、分娩后的血清置-80℃冻存;采用多重荧光微球技术,检测IL-2、IFN-γ、IL-6、IL-8、IL-10、IL-12、IP-10、TNF-α等8种SLE相关细胞因子。结果细胞因子IFN-γ、IL-6、IL-8、IL-10、IL-12、IP-10、TNF-α在SLE妊娠妇女孕早期、中期、晚期及产后的血清水平较健康对照组各产程高,但仅3种细胞因子IL-8、IL-10、IP-10差异存在统计学意义(P<0.05)。同时,这3种细胞因子在活动期SLE妊娠组孕早期、中期、晚期及产后的血清水平,均较稳定期SLE妊娠组各产程高,差异存在统计学意义(P<0.05)。结论细胞因子的表达在SLE妇女对妊娠的反应变化情况与正常妇女相似,其中SLE相关因子IL-8、IL-10、IP-10在各产程较正常妊娠妇女偏高,特别是在活动期SLE患者妊娠时,IL-8、IL-10、IP-10水平与SLE患者的狼疮病情活动度成正相关,可以作为SLE活动的指标,为临床监测作指导。 Objective To investigate the interactions of cytokines in systemic lupus erythematosus(SLE)patients withpregnancy,to provide experimental references for cytokines as monitoring indicators of SLE disease activity and prognosis ofpregnancy. Methods Blood samples were taken from 30 pregnant women with SLE and 30 normal pregnant women at first,second and third trimesters respectively,and after delivery,and then were preserved at-80℃. The levels of 8 SLE-relatedcytokines,IL-2,IFN-γ,IL-6,IL-8,IL-10,IL-12,IP-10 and TNF-α were measured in the same time by multiplexedfluorescent microsphere technology. Results The levels of IFN-γ,IL-6,IL-8,IL-10,IL-12,IP-10 and TNF-α were higherin the SLE group than normal group at first,second and third trimesters and after delivery,respectively,but only IL-8,IL-10 and IP-10 were significantly higher(P〈0.05). Moreover,their levels were also higher in the active SLE women than stable SLEwomen at every trimester(P〈0.05). Conclusion The expression of cytokines is similar in pregnant women with SLE as innormal women. But the levels of lupus related cytokines IL-8,IL-10 and IP-10 are higher in SLE pregnant women than normalones in every trimester,especially in the patients with active lupus. The levels of IL-8,IL-10 and IP-10 in patients with SLEare positive correlated with SLE disease activity index,so they can be used as lupus activity index for clinical monitoring.
