摘要
目的探讨参芪地黄汤辅助治疗狼疮性肾炎(LN)气阴两虚证的效果。方法选取2022年1月—2023年1月山亭区人民医院收治的78例LN气阴两虚证患者为研究对象,按随机数字表法将其分为对照组及观察组,每组39例。对照组采用西药治疗,观察组在此基础上采用参芪地黄汤治疗,均持续治疗3个月。比较两组患者的临床疗效、肾功能指标、肾损伤标志物、中医证候评分及不良反应发生情况。结果观察组的治疗总有效率为94.87%,高于对照组的79.49%,差异有统计学意义(P<0.05)。治疗后,观察组的尿素氮、血肌酐、24 h尿蛋白量水平均低于对照组,组间差异有统计学意义(P<0.05)。治疗后,观察组可溶性血栓调节蛋白、中性粒细胞明胶酶相关脂质运载蛋白水平均低于对照组,可溶性肿瘤坏死因子Ⅰ型受体水平高于对照组,组间差异有统计学意义(P<0.05)。治疗后,观察组的倦怠乏力、少气懒言、口燥咽干等中医证候评分均低于对照组,组间差异有统计学意义(P<0.05)。两组的不良反应发生率比较,差异无统计学意义(P>0.05)。结论参芪地黄汤可提高LN气阴两虚证患者的治疗效果,减轻肾损伤,加快肾功能恢复,降低中医证候评分,安全可靠。
Objective To investigate the effect of Shenqi Dihuang Decoction in the treatment of lupus nephritis(LN)with Qi-Yin deficiency syndrome.Methods 78 LN patients with Qi-Yin deficiency syndrome admitted to Shanting District People's Hospital from January 2022 to January 2023 were selected as the study objects and were divided into a control group and an observation group according to random number table method,with 39 cases in each group.The control group was treated with western medicine,and the observation group was treated with Shenqi Dihuang Decoction on this basis.Both groups were treated for 3 months.The clinical efficacy,renal function indexes,renal injury markers,TCM syndrome scores and occurrence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 94.87%,higher than 79.49%of the control group,the difference was statistically significant(P<0.05).After treatment,the levels of blood urea nitrogen,serum creatinine and 24 h urinary protein in the observation group were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).After treatment,the levels of soluble thrombomodulin and neutrophil gelatinase-associated lipocalin in the observation group were lower than those in the control group,and the level of soluble tumor necrosis factor receptor-I was higher than that in the control group,and the differences between the groups were statistically significant(P<0.05).After treatment,the traditional Chinese medicine syndrome scores of fatigue,lack of Qi and no desire to speak,dry mouth and throat in the observation group were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Shenqi Dihuang Decoction can improve the therapeutic effect of LN patients with Qi-Yin deficiency syndrome,alleviate kidney injury,accelerate renal function recovery,reduce TCM syndrome scores,and is safe and reliable.
