摘要
目的观察补肾促排方治疗多囊卵巢综合征排卵障碍性不孕的临床疗效及可能机制。方法收集多囊卵巢综合征排卵障碍性不孕患者56例,随机分为治疗组和对照组各28例。治疗组给予补肾促排方口服,从月经第5天开始,每日1剂,B超监测排卵后停服,如无排卵则服至下次月经来潮时。对照组从月经第5天开始口服枸橼酸氯米芬胶囊,每次50 mg,每日1次,共5天。两组均治疗3个月经周期。观察两组患者治疗前后卵泡大小、子宫内膜厚度及卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E_2)、睾酮(T)水平,于治疗后计算排卵率与妊娠率判定临床疗效。结果治疗组临床疗效总有效率(排卵率)为82.14%,对照组为78.57%,两组疗效比较差异无统计学意义(P>0.05),治疗组治愈率(妊娠率)明显优于对照组(P<0.05)。治疗组治疗后排卵期卵泡大小、子宫内膜厚度及E_2水平均较治疗前明显升高,LH、T水平均较治疗前明显下降(P<0.05);对照组治疗后排卵期卵泡大小较治疗前明显升高,子宫内膜厚度及E_2、LH、T水平均较治疗前明显下降(P<0.05)。治疗后治疗组子宫内膜厚度及E_2水平均明显高于对照组,LH、T水平均明显低于对照组(P<0.05或P<0.01)。结论补肾促排方治疗多囊卵巢综合征排卵障碍性不孕可获得较好的妊娠率,可能与改善卵泡发育及生殖激素水平有关。
Objective To examine the clinical effect and possible mechanism of Bushen Cupai Decoction（ 补肾促排方） treating polycystic ovarian syndrome（ PCOS） ovulation dysfunction infertility. Methods Fifty-six PCOS ovulation dysfunction infertility patients were collected and randomized into treatment group（ 28 cases） and control group（ 28 cases）. The treatment group was given Bushen Cupai Decoction orally from 5th day of menstruation,one dose every day. Medication stopped after ovulation by type-B ultrasonic monitoring. If there was no ovulation,patients should take the decoction until next menstruation. The control group was given clomifene citrate capsules orally from5 th day of menstruation,50 mg each time,once every day,for 5 days. Both groups were treated for 3 menstrual cycles. The size of folliole,the thickness of endometrium,as well as the level of follicle stimulating hormone（ FSH）,luteinizing hormone（ LH）,estradiol（ E_2） and testosterone（ T） of patients in both groups were examined before and after treatment. The effect was evaluated by calculating ovulation rate and pregnancy rate after treatment. Results The total effective rate（ ovulation rate） was 82. 14% and 78. 57% in the treatment group and control group respectively. The difference was not significant（ P〈0.05）. The cure rate（ pregnancy rate） in the treatment group was superior to that in the control group（ P〈0. 05）. After treatment,the size of folliole,the thickness of endometrium and the level of E_2 in the treatment group increased and the level of LH and T decreased（ P〈0. 05）. After treatment,the thickness of endometrium and the level of E_2 in the treatment group were much higher than those in the control group. The level of LH and T were much lower（ P〈0. 05 or P〈0. 01）. Conclusion Bushen Cupai Decoction might obtain good pregnancy rate in treating PCOS ovulation dysfunction infertility. It might relate to improve follicular development and the level of reproductive hormones.
