摘要
"十八反"是中药用药配伍禁忌原则,然而古今均有含"十八反"反药组合的中药方剂/中成药,为开展对"十八反"原理的进一步深入研究工作,对44个含"十八反"反药组合的中成药进行文献检索、归类、整理和分析。笔者发现已经开展相关研究的有27个中成药,以临床疗效观察和病例分析最多,其次为质量标准理化研究,偶见炮制剂型研究和不良反应报道,文献研究和动物实验较少,未见流行病调查、大数据分析,研究中中医药的理论特色体现不足,针对同一个中成药的研究不够深入和系统,缺乏对"十八反"反药组合配伍应用的毒性及副作用相关研究,对已见不良反应的药物,未针对其原因是否为反药同用,开展更多研究,以上均应为未来研究的重点。
‘Eighteen incompatible medicaments' is a sort of compatibility taboo principle of Chinese materia medica. However, there are lots of Chinese patent medicine which contain eighteen incompatible medicaments herb pairs from ancient times to the present. In order to review and make a comment about these kinds of Chinese patent medicine, solve disputed historical issue, and promote deep research related to eighteen incompatiblemedicaments, we carried on the document retrieval, sorting and analysis about 44 kinds of Chinese patent medicine containing herb pairs of eighteen incompatible medicaments. We found that 27 Chinese patent medicines containing eighteen incompatible medicaments herb pairs had been carried out for research. Most of these researches were related to clinical effect observation and case analysis, followed by quality standard of Chinese patent medicine via physicochemical method. A few were related to processing preparation research and adverse reaction report. Literature research and animal experiment were rarely searched. None of epidemiological survey, large data analysis or systematic review had been found. In these researches, the theoretical characteristics of Chinese medicine were not fully reflected. For the same Chinese patent medicine, the researches were not in-depth and systematic enough. There was very few of study on the toxicity and side effect of the herb pairs of eighteen incompatible medicaments inside these Chinese patent medicines, and almost nobody focused on the reason of theadverse reaction, which probably could be caused by combination of opposing herbal remedies in patent medicines. All above issues should be thefocus of future research.
