摘要
文章从分析疾病医学的局限性及面临的困境着眼,指出精准医学的意义不是基于疾病医学的精准,其核心是建立一个能个性化地描述人体状态并实现个性化调控的状态医学体系。而一直不被西方医学乃至西方科学理解的中医学,本质上就是一个状态医学体系。与今天美国精准医学着眼于基因组、蛋白组、代谢物组等研究,从微观层面建立人体状态描述不同,中医学的人体状态描述是基于整体层面建立的。近年来,随着复杂性科学的进展,生物体内"测不准原理"被揭示,还原论医学的"分析-重构"模式从理论和实践角度均被证明是不可行的。因此,状态医学建立人体整体的状态描述只能采用中医学的模式,从对人体整体层面行为功能的考察入手。而基于整体层面的状态医学,无论从状态描述系统的简单性、对人体个性化状态把握的精准性、寻找从宏观层面调控人体的药物以及对人体状态调控的整体性与简单性等方面,均有着从微观层面建立人体的状态描述所无法比拟的优势。而引入现代医学的检测手段与人工智能、大数据分析技术,在中医学基础上,建立起基于整体层面的精准医学体系,不仅可行,而且势必成为精准医学实现远期目标的最佳途径。由此,未来的医学体系,将是以基于整体的状态医学——整体医学,与融汇中西医疾病分类体系的疾病医学相结合的、建立在科学实证基础上的医学体系。
This paper analyses the limitation of pathology and the dilemma facing it.It points out that the pursuit of precision medicine does not rely on the accuracy of pathology.Its core is to establish a status medical system that may describe the state of human body to actualize an individualized control status medical system.Traditional Chinese medicine(TCM),has all along not been comprehended by the western science and western medicine,in essence is a status medicine system.Today the precision medicine in the United States embarks on the micro level of body status focusing on the researches of genome,proteome,and metabolite groups.Whereas TCM approaches from a holistic perspective.In recent years,with the progress of the complexity science theory and the revelation of the uncertainty principle,it has been proven that the 'analysisreconstruction' mode in reductionism is impractical from both theoretical and practical points of view.Therefore,status medicine may deploy TCM mode to examine the overall state of the functional level of the human body from a holistic approach.A status medicine that bases on the holistic approach can best describe the state of the human body in a simple manner and thus may devise a simple and accurate individualized medicines and to regulate the whole body in a simple manner.The micro level approach to treat a person lags behind the advantages of the holistic approach.With the introduction of modern medical tests and examinations,artificial intelligence,and big data couple with the fundamentals of TCM,we are forge ahead on our way to actualize precision medicine.Thus,the future medical system will base on an overall status medicineholistic medicine that conjugates modern medicine and TCM.And it will be a medical system that builds on the basis of the scientific principles.
