摘要
目的总结睾丸结核的彩色多普勒超声声像表现,分析误诊原因。方法回顾性分析18例睾丸结核患者(单侧病变15例,双侧病变3例)临床及彩色多普勒超声声像资料。结果彩色多普勒超声诊断睾丸结核15例(单侧病变12例,双侧病变3例),诊断符合率85.7%;超声声像表现为弥漫结节型12例,肿块型2例,脓肿型3例,窦道型1例;术前超声误诊睾丸炎1例、睾丸精原细胞瘤1例、睾丸囊肿1例,超声表现分别为弥漫结节型、肿块型、脓肿型。结论睾丸结核彩色多普勒超声声像表现具有一定特异性,但与其他睾丸疾病声像图表现有重叠,临床应结合病史及其他检查结果以提高其超声诊断准确率。
Objective To analyze the reason of misdiagnosis of testicular tuberculosis by summarizing the color Doppler ultrasound imaging features of testicular tuberculosis. Methods The clinical and ultrasound data of 18 patients with testicular tuberculosis including 15 patients with unilateral lesion and 3 patients with bilateral lesion were retrospectively analyzed. Results In 18 patients, 15 patients were diagnosed testicular tuberculosis by ultrasound including 12 patients with unilateral lesion and 3 patients with bilateral lesion, and the diagnosis accordance rate was 85.7%. The ultrasound showed diffuse nodule type in 12 patients, mass type in 2 patients, abscess type in 3 patients and sinus type in 1 patient. The preoperative ultrasound misdiagnosed orchitis in 1 patient, seminoma in 1 and testicular cyst in 1, showing diffuse nodule type, mass type and abscess type, respectively. Conclusion Testicular tuberculosis has certain features on color Doppler ultrasound, which has overlap with the other testicular diseases. The clinical history and other examination results should be considered together to raise the diagnostic accuracy for testicular tuberculosis.
