摘要
目的观察参芪扶正注射液对进展期胃癌患者免疫功能的影响。方法进展期胃癌患者82例,分为对照组40例及观察组42例,2组患者均采用ECF-L方案化疗,观察组在此基础上加用参芪扶正注射液。化疗2个周期后评价2组近期疗效、不良反应及生活质量改善情况,并比较2组化疗前1天及化疗2个周期后外周血CD_3^+、CD_4^+、CD_8^+T细胞亚群比例及NK细胞比例。结果所有患者均完成2个周期的治疗,观察组客观缓解率(ORR)为40.5%,对照组ORR为37.5%。2组ORR差异无统计学意义(P>0.05)。观察组生活质量改善率64.3%显著高于对照组的37.5%(P<0.05)。观察组化疗2个周期后CD_3^+、CD_4^+T细胞亚群、NK细胞水平、CD_4^+/CD_8^+比例均高于化疗前1天,且高于对照组,差异均有统计学意义(P<0.01)。观察组骨髓抑制、消化道反应、神经毒性发生率显著低于对照组(P<0.05)。结论参芪扶正注射液能够改善进展期胃癌化疗患者生活质量及免疫功能,减轻化疗不良反应。
Objective To observe the effect of shenqi fuzheng injection on immune function of patients with advanced gastric cancer. Methods A total of 82 patients with advanced gastric cancer were divided into control group（ n = 40） and observation group（ n = 42）. Both groups were given ECF-L regimen,and observation group was treated with shenqi fuzheng injection. The short-term efficacy,adverse reactions and quality of life（ QOL） were evaluated in two groups after two-cycle chemotherapy. The proportions of peripheral CD_3~＋,CD_4~＋and CD_8~＋and natural killer（ NK） cells were compared between two groups one day before chemotherapy and after two-cycle chemotherapy. Results All patients completed two cycles of treatment. The objective response rate（ ORR） was40. 5% in observation group and 37. 5% in control group（ P〈0. 05）. The ameliorative rate of QOL was significantly higher in observation group（ 64. 3%） than control group（ 37. 5%）（ P〈0. 05）.The proportions of CD_3~＋,CD_4~＋NK cells and CD_4~＋/ CD_8~＋in observation group after two-cycle chemotherapy were significantly higher than those on one day before chemotherapy and those in control group after two- cycle chemotherapy（ P〈0. 01）. The incidence rates of myelosuppression,gastrointestinal reactions and neurotoxicity were all significantly lower in observation group than control group（ P〈0. 05）. Conclusion Shenqi fuzheng injection can improve the immune function and QOL,and reduce the toxic and adverse reactions of patients with advanced gastric cancer.
