丙泊酚抑制失血性休克复苏后大鼠肠道黏膜细胞凋亡

The protective effect on intestine by propofol after hemorrhagic shock and resuscitation

摘要 目的:研究失血性休克复苏后应用丙泊酚对肠道黏膜的保护作用。方法:将24只Wistar大鼠随机分为3组,分别为丙泊酚组(P组)、失血性-休克复苏组(HS-R组)、假手术组(C组),每组8只。P组、HS-R组在复苏后分别持续泵入丙泊酚10mg/(kg·h)、生理盐水1ml/(kg·h),持续泵入2h后开腹取小肠组织约2cm做石蜡切片,采用免疫组织化学法测肠道组织中凋亡蛋白/抗凋亡蛋白(Bcl-2/Bax)表达水平,采用TUNELL法检测肠道组织细胞凋亡水平。结果:HS-R组和P组的Bax表达水平显著高于C组,P组的Bax表达水平显著低于HS-R组(P<0.01);HS-R组和P组的Bcl-2表达水平显著低于C组,P组Bcl-2表达水平显著高于HS-R组(P<0.01);P组和HS-R组凋亡指数均显著高于C组,P组凋亡指数显著低于HS-R组(P<0.01)。结论:失血性休克复苏后观察肠道黏膜有明显损伤,丙泊酚可通过调节Bcl-2/Bax表达水平来减轻肠黏膜损伤。 Objective:To study the protective effect of propofol on intestine after hemorrhagic shock and resuscitation.Method:Twenty-four Wistar rats were randomized into 3groups:propofol group(P group,n=8),resuscitation group(HS-R group,n=8)and sham operation group(C group,n=8).Following resuscitation,P group was infused propofol at 10mg/(kg·h)and HS-R group was infused normal saline at 1 ml/(kg·h)for two hours.Taken 2cm intestinal tissue to do paraffin sections using immunohistochemical staining method to measure Bcl-2/Bax protein level,and TUNELL method to measure the level of intestinal cell apoptosis.Result:Bcl-2protein levels in HS-R group and P group was lower than those in C group.Bcl 2protein in P group was higher than HS-R group(P〈0.01).The expression of Bax protein in the HS-R group and P group were higher than C group.Expression of Bax in P group was lower than HS-R group(P〈0.01).The intestinal tissue apoptosis index in HS-R group and P group were higher than those in C group.The intestinal tissue apoptosis index in P group was lower than HS-R group(P〈0.01).Conclusion:The intestinal tissue was damaged after hemorrhagic shock and resuscitation.Propofol can regulation the balance of apoptosis and anti-apoptotic proteins to reduce intestinal tissue injury.
