摘要
目的分析右美托咪定对肝叶切除患者围术期肝功能及血浆细胞因子水平的影响,从而为临床治疗提供依据。方法选取2012年1月至2015年1月承德医学院附属医院和承德市宽城县医院行肝叶切除术患者80例为研究对象,分为观察组与对照组,每组40例,观察组注射右美托咪定1μg/kg,注射时间为10min,然后以0.5μg·kg-1·h-1持续输注,对照组注射等量0.9%氯化钠注射液。结果 2组患者肝门阻断时间、手术时间以及术中输血量差异无统计学意义(P>0.05);2组患者在T0期,丙氨酸转氨酶(ALT)和天冬氨酸转氨酶(AST)差异无统计学意义,随着时间的延长,2组ALT与AST均显著升高,但对照组升高更加明显,差异有统计学意义(P<0.05);2组患者在T0期,肿瘤坏死因子(TNF)-α和白细胞介素(IL)-10差异无统计学意义,随着时间的延长,2组TNF-α和IL-10均显著升高,但对照组升高更加明显,差异有统计学意义(P<0.05)。结论右美托咪定可以抑制肝功能损伤和细胞因子的释放,有利于降低缺血再灌注损伤。
Objective To analyze influence of dexmedetomidine on liver function and cytokine levels in perioperative period of patients undergoing hepatectomy,thereby providing a basis for clinical treatment.MethodsEighty patients were selected from Chengde Medical College Hospital and Hospital of Kuancheng County Chengde City from January 2012 to January 2015,and were divided into observation group(40cases)and control group(40cases)randomly.Observation group were injected with dexmedetomidine(1μg/kg),and the injection lasted 10 minutes.Then continuous infusion(0.5μg·kg^-1·h^-1)was conducted 0.5μg·kg^-1·h^-1.The control group were injected with normal saline.Results Patients pringle time,operation time and intraoperative blood transfusion of two groups was no significant difference(P〉0.05);ALT,AST,TNF-αand IL-10 of two groups at T0 period were no differences,with time,control group were significantly increased(P〈0.05).Conclusion Dexmedetomidine can inhibit liver dysfunction and release of cytokines,help to reduce ischemia-reperfusion injury.
