摘要
目的:应用脑性瘫痪《国际功能、残疾和健康分类(儿童与青少年版)》(International Classification of Functioning,Disability and Health,Children and Youth Version,ICF-CY)核心分类组合简明通用版描述脑瘫儿童功能,研究其临床实用价值。方法:脑性瘫痪儿童40例,于入院24h内分别采用脑性瘫痪ICF-CY核心分类组合简明通用版、PEDI量表进行评定。检测脑性瘫痪ICF-CY核心分类组合简明通用版重测信度和评定者间信度均采用Kappa检验,检测内部一致性信度采用Cronbachα系数,检测与PEDI的同步效度采用Spearman相关系数。结果:脑性瘫痪ICF-CY核心分类组合简明通用版身体功能、活动与参与、环境因素三个领域的24个类目的重测信度kappa值为0.856—1.000,信度优;身体功能、活动与参与、环境因素三个领域的24个类目的评定者间信度kappa值为0.696—0.900,信度中到优;身体功能、活动与参与、环境因素三个领域的24个类目内部一致性信度Cronbachα系数>0.7,有较好的内部一致性;身体功能、活动与参与、环境因素三个领域的24个类目与PEDI有负强相关性(P<0.01)。结论:采用脑性瘫痪ICF-CY核心分类组合简明通用版身体功能、活动与参与、环境因素3个领域描述脑性瘫痪儿童的功能可靠、有效,具有良好的临床实用价值。
Objective： To explore the application of Brief Common ICF Core Set for Children and Youth with Cerebral Palsy in rehabilitation assessment for Children with Cerebral Palsy. Method： Forty children with cerebral palsy were recruited as participants who had been assessed by Brief Com- mon ICF Core Set for Children and Youth with Cerebral Palsy and pediatric evaluation of disability inventory （PEDI） within 24 hours of admission. The test-retest reliability and interrater reliability were analyzed by calcu- lating kappa correlation statistics. The internal consistency reliability was analyzed by calculating Cronbach＇s ct coefficient. The concurrent criterion validity was analyzed by calculating Spearman＇s rank correlation coefficients. Result： The 24 categories of 3 domains （Rodv Functions_ Activities and Particination_ Envirnnmental Pactnr of Brief Common 1CF Core Set for Children and Youth with Cerebral Palsy had excellent test-retest reliability （Kappa value 0.856-1.000）. The 24 categories of 3 domains had moderate to excellent interrater reliability （Kappa value 0.696-0.900）. The 24 categories of 3 domains had a better internal consistency reliability （Cron- bach a coefficient 〉0.7）. The total scores of 24 categories of 3 domains and the scores of PEDI had a high inverse correlation of Spearman＇s rank correlation coefficients （P〈0.01）. Conclusion： Brief Common ICF Core Set for Children and Youth with Cerebral Palsy can be used as a tool to describe the function of children with cerebral palsy.
