摘要
原发性老年肌肉衰减综合征(Sarcopenia)是一种随年龄增加以骨骼肌质量下降和骨骼肌力量或功能下降为主要特征的退行性综合征。肌肉衰减综合征已成为严重影响中老年人正常生活和自理能力的一个突出问题。随增龄发生的肌纤维萎缩、骨骼肌蛋白合成减少、骨骼肌线粒体功能紊乱等都可能是导致老年个体发生肌肉衰减综合征的原因[1]。其中全身性炎症水平亢进,生长激素信号系统衰减以及激素水平的下降是其发生的主要机制。肌肉卫星细胞的数量减少和功能减弱,多种细胞内信号通路以及细胞内泛素蛋白酶系统也参与肌肉衰减综合征的发生[2]。中医认为肌肉衰减综合征属于"痿证""虚劳""体惰"范畴。其病位在筋脉肌肉,其发生主要与肝脾肺肾有关,脾主肌肉四肢,因而与脾关系最为密切。《黄帝内经》中提出"治痿独取阳明"的理论,《素问·痿论》载:"阳明者,五脏六腑之海,主润宗筋,宗筋主束骨而利机关也",阳明胃经起到了关键的作用。
Primary attenuation syndrome（ Sarcopenia） is a degenerative syndrome characterized with the quality and strength of the skeletal muscle decrease with aging. It has become a serious impact on the elderly patients＇ normal life and ability to care for themselves. We have gradually learned the Saccopenia mechanisms： with aging,the muscle fiber atrophy occurs in skeletal muscle protein synthesis and reduced skeletal muscle mitochondrial dysfunction and so on,may leading to Sarcopenia. Furthermore,loss in function and number of muscle satellite cells are another mechanism. Multiple signaling pathways as well as the ubiquitin-proteasome system is also related to the development of Sarcopenia. TCM holds that the muscle attenuation syndrome belongs to Wei syndrome,Xulao and Tiduo. The disease locates in tendons and muscle and it is mainly related to the liver,spleen and kidney. Spleen dominates limbs and muscle,and thus it has the most closely relationship with the spleen. And it is put forward that treating Wei syndrome lonely taking Yang Ming theory in Huangdi Neijing： ＂Yang Ming is the sea of Zang-fu organs and dominates nourishing the tendons which mainly dominate bone and joints,playing a key role. ＂
