摘要
目的:对胸部肿瘤三维适形放疗急性放射性肺损伤相关因素开展分析。方法:随机选取我院2013年1月至2014年12月间收治的开展三维适形放疗治疗的胸部肿瘤患者200例,对患者的放疗开展时间、糖尿病史、心血管疾病史、年龄、性别、出现急性放射性肺损伤的时间等一般资料进行记录,并要对其放疗的总剂量,双肺的V5、V10、V20、V30及双肺所应用的平均剂量开展统计分析。结果:所选的200例应用三维适形放疗治疗的胸部肿瘤患者中,有30例患者发生了放射性肺损伤,其总的发生率为30%,其中有16例患者表现为急性放射性肺损伤,其所占比例为53.33%,导致患者发生急性放射性肺损伤的危险因素为:v30≥19%,V20≥27%,V10≥41%,v5≥56%;并且通过分析可以看出,当患者的总剂量大于等于56Gy时,患者发生急性放射性肺损伤的概率明显加大。结论:应用三维适形放疗开展治疗的胸部肿瘤患者发生急性放射性肺损伤的相关因素主要有:V30、V20、V10、V5及放疗总剂量。
Objective: Chest tumor dimensional conformal radiotherapy-related factors to carry out analysis of acute radiation-induced lung injury.Methods: Randomly selected in our hospital between January 2013 to December 2014 200 cases of breast cancer patients admitted to carry out three-dirnemional conformal radiation therapy, the patient's radiation carried out time, history of diabetes, history of cardiovascular disease, age, sex, acute general information on radiation-induced lung injury in time to record, and you want to total dose of radiotherapy, lungs of V5, V10, V20, V30 and lungs average dose applied to carry out statistical analysis.Results: Breast cancer patients selected 200 cases of application of three- dimensional confonnal radiation therapy, 30 patients developed radiation-induced lung injury, the total incidence of 30%, including 16 patients with manifestatiom of acute radiation-induced lung injury, it the proportion of 53.33%, which leads to the occurrence of acute radiation-induced lung injury in patients with risk factors as: V30 ≥ 19%, V20 ≥ 27%, V10 ≥ 41%, V5 ≥ 56%; and through the analysis can be seen, when the total patients at doses greater than or equal probability 56Gy, patients with acute radiation-induced hmg injury significantly increased.Conclusion: Application-related factors to carry out three-dimensional conformal radiation therapy of breast cancer patients with acute radiation-induced lung injury are mainly: V30, V20, V10, V5, and the total radiation dose.
