摘要
[目的]研究3D打印技术应用于治疗高能量Pilon骨折的临床疗效。[方法]选取64例高能量Pilon骨折患者,随机分为试验组及对照组,试验组应用3D打印技术打印出Pilon骨折实物模型,进行术前设计及制定个体化手术方案后再进行手术;对照组按常规方法行切开复位内固定术。记录两组患者手术时间、术中出血量;采用Burwell-Charnley评价标准评价骨折复位情况;并采用olerud-Molander踝关节功能主观评分标准分别评价患者术后6、12个月踝关节功能恢复情况。[结果]试验组手术时间、出血量明显小于对照组,差异有统计学意义(P<0.05);试验组患者得到解剖复位的例数显著多于对照组,差异有统计学意义(P<0.05);对所有患者进行随访,术后6、12个月试验组患者踝关节功能评分均高于对照组,差异有统计学意义(P<0.05)。[结论]应用3D打印技术辅助治疗高能量Pilon骨折大大节约了手术时间,减少了术中出血量,提高了骨折解剖复位率,经过长时间随访踝关节功能恢复疗效满意。
[ Objective ] To investigate the clinical therapeutic effects of three - dimensional priming technique in tibial Pi- Ion fractures caused by high - energy. [ Method] A total of 64 patients with tibial Pilon fractures caused by high - energy were assigned into experimental group and control group. Patients in the experimental group received operation under the assistance of three - dimensional printing technique including printing the physical models of Pilon fractures, designing preoperatively and for- mulating individualized operative plan on them. Those in the control group were traditional treated with open reduction and inter- hal fixation surgery. The surgical time and blood loss of patients during operation were compared. The situation of fracture reduction after surgery was evaluated according to the radiological evaluation criteria (Burwell - Charnley standard ). At 6 months and 12 months after the operation, the function of an- kle joint was evaluated according to the clinical evaluation criteria( Olerud -Molander subjective scoring standard). [ Result] The surgical time and blood loss during operation in the experi- mental group was less than in the control group, the difference between the groups was statistically significant (P 〈 0.05 ). The anatomic reduction rate in the experimental group was notablely higher than the control group, the difference between the groups was statistically significant(P 〈 0.05). All patients were followed over time, the functional scoring of ankle joint in the experi- mental group was higher than the control group at 6 months and 12 months after surgery,the difference between the groups was statistically significant( P 〈 0.05 ). [ Conclusion] The application of three -dimensional printing technique in the treatment of the tibial Pilon fractures caused high - energy can reduce the surgical time and blood loss during operation. It also can improve the degree of fracture reduction, and the perfect functional recovery of ankle joint can be reached after a long time follow - up postoperatively.
