摘要
【目的】探讨脑梗死患者经抗血小板二级预防后再发脑梗死的临床特征及危险因素。【方法】选择本院神经内科2011年12月至2012年12月收治的初发脑梗死患者152例作为研究对象，随访2年，随访期间所有患者均规律服用阿司匹林进行抗血小板二级预防，根据随访期间有无再发脑梗死分为再发组与无再发组，对再发脑梗死进行单因素和多因素非条件Logistic回归分析。【结果】152例患者共纳入研究147例，再发脑梗死34例（23．13％）；单因素分析结果显示，糖尿病、高脂血症、高同型半胱氨酸血症对脑梗死患者经抗血小板二级预防后再发脑梗死有显著影响（P〈0．05）；多因素非条件Logistic回归分析结果显示，血清同型半胱氨酸（Hcy）偏高、血糖水平偏高、低密度脂蛋白胆固醇（LDL-C）及总胆固醇（TC）或3项偏高是脑梗死患者经抗血小板二级预防后再发脑梗死的独立危险因素（P〈0．05）。【结论】血清Hcy偏高、血糖水平偏高、LDL-C及TC偏高或3项偏高是脑梗死患者经抗血小板二级预防后再发脑梗死的独立危险因素。
Objective1 To investigate the clinical features and risk factors of cerebral infarction recurrence in patients after anti platelet grade two therapy.[Methods] 152 patients with cerebral infarction from December 2012 to December 2011 were selected as the research subjects, and all patients were followed up for 2 years. During the follow-up period, all of them were treated with aspirin for anti platelet. According to the recurrence or not, the patients were divided into recurrent group（the research group） and non recurrent group （the control group）.The sin- gle factor and multiple-factor Logistic regression analysis were performed in the research group. [Results]147 cases of 152 patients were included, among which 34 cases of cerebral infarction （23.13%） recurred Single factor analysis showed that diabetes, hyperlipidemia, high homocysteine levels are main factors for cerebral infarction recurrence after antiplatelet grade two therapy （P 〈0.05） Multivariate Logistic regression analysis showed that high homocysteine levels （Hcy）, high blood glucose levels, low density lipoprotein cholesterol （LDL）, total cholesterol （TC） （ or the conventional three high included ） were the independent risk factors in the patients with recurrent cerebral infarction by anti platelet grade two（ P 〈0.05）.[Conclusion]The levels of serum Hcy, high blood glucose, LDL and TC （or the conventional three high ） were the independent risk factors for cerebral infarction recurrence after anti platelet grade two therapy.
