抗菌药物输注时间与剂量对肛肠术后感染的影响研究 被引量:13

Influence of time and dose of antibiotics infusion on infections after anorectal surgery

摘要 目的探讨肛肠外科围手术期抗菌药物输注时间和剂量对术后感染的影响,为临床用药提供参考。方法选取术后给药方法相同的肛肠外科手术患者328例,其中A组161例术前不使用抗菌药物,B组167例术前30min^2h给予抗菌药物静脉滴注;选取术前给药方法相同的肛肠外科手术患者518例,其中a组166例术后给药≤3d,b组177例术后给药4~7d,c组175例术后给药≥8d;选取围手术期给药时间相同的肛肠外科手术患者309例,其中小剂量组161例,每次给药0.2g,大剂量组148例,每次给药0.3g;对比各组患者术后感染率及切口愈合情况。结果患者术后感染率及甲级愈合率A组分别为3.11%和96.89%,B组患者分别为0.60%和99.40%;a组患者分别为3.61%和96.39%,b组患者分别为1.13%和98.87%,c组患者分别为0.57%和99.43%;小剂量组患者分别为1.24%和98.76%,大剂量组分别为0.68%和99.32%;B组患者术后感染率明显低于A组,甲级愈合率明显高于A组;a组患者术后感染率明显高于b、c两组,甲级愈合率明显低于b、c两组,差异有统计学意义(P<0.05);b、c两组,小剂量和大剂量两组患者术后感染率及甲级愈合率差异无统计学意义。结论术前30min^2h单次给药,术后每天两次连续给药4~7d,每次给药200ml 0.2g是一种较为合理用药方案。 OBJECTIVE To explore the influence of the time and dose of perioperative antibiotics infusion on postoperative infections in the patients undergoing anorectal surgery so as to provide guidance for clinical use of antibiotics.METHODS A total of 328 anorectal surgery patients who were treated with same postoperative drug administration were enrolled in the study,the group A with 161 cases was not treated with antibiotics before the surgery,the group B with 167 cases was given the intravenous drip of antibiotics at 30min-2hours before the surgery.Totally 518 anorectal surgery patients who were treated with the same drug administration before the surgery were included,the group a with 166 cases was treated with antibiotics for no more than 3days after the surgery,the group b with 177 cases was treated with antibiotics for 4-7days after the surgery,and the group c with 175 cases was treated with antibiotics for no less than 8days after the surgery.Totally 309 anorectal surgery patients who had the same perioperative drug administration time were enrolled in the study,the low-dose group with 161 cases was given the dosage of 0.2g for each time,and the high-dose group with 148 cases was treated with the dosage of0.3g for each time.The incidence of postoperative infections and incision healings were observed and compared.RESULTS The incidence of postoperative infections was 3.11%in the group A,0.60%in the group B,3.61%in the group a,1.13%in the group b,0.57%in the group c,1.24%in the low-dose group,0.68%in the highdose group.The rate of grade A incision healing was 96.89%in the group A,99.40%in the group B,96.39%in the group a,98.87%in the group b,99.43% in the group c,98.76% in the low-dose group,99.32% in the high-dose group.The incidence of postoperative infections of the group B was significantly lower than that of the group A;the rate of grade A incision healing of the group B was significantly higher than that of the group A.The incidence of postoperative infections of the group a was significantly higher than that of the group b and the group c;the rate of grade A incision healing of the group a was significantly lower than that of the group b and group c,with significant difference(P〈0.05).There was no significant difference in the incidence of postoperative infections or the rate of grade A incision healing between the group b and group c or between the low-dose group and the high-dose group.CONCLUSIONIt is a reasonable medication program to conduct the single drug administration at 30min-2hours before the surgery,successive drug administration twice per day for 4-7days after the surgery,and drug administration with dosage of 200 ml 0.2g for each time.
