摘要
目的:了解现阶段上海市普陀区社区医院卫生服务政策执行情况。方法:选取上海市普陀区某社区卫生服务中心周边20所社区医院,调查分析各社区医院开办性质及基本药物目录、药品零差价销售制度等执行情况。结果:20所社区医院中,城镇职工定点医疗保险开放率70%,城镇居民医疗保险开放率65%,新型农村合作医疗开放率10%,新农村合作医疗开放率相对较低(P<0.05)。13所医院采用了收支两条线管理方式。服务中心对国家基本药物制度执行率为100.0%,卫生服务站执行率84.6%,差异有统计学意义(P<0.05)。国家基本药物目录执行率75%,省级增补目录执行率80%,地市增补目录执行率45%,国家基本药物目录、省级增补目录执行率均高于地市增补目录执行率(P<0.05)。20所社区医院中,17所执行了药品零差价销售制度,执行率为85%。结论:社区医院在药物目录、零差价销售制度、基本药物制度等方面的落实工作还有提升空间,应着重完善农村合作医疗落实工作,提高社区医院服务质量。
Objective： To study the implementation situation of the health service policy in Putuo district community hospitals in Shanghai at present. Methods ：20 community hospitals around Putuo district in Shanghai were selected to investigate and analyze the nature of the community hospitals and the implementation situation of medical insurance, essential drug list and the pharmaceutical retail sales price. Results： Among these 20 community hospitals, the open rate of the basic medical insurance for urban workers, the basic medical insurance for urban residents and the new rural cooperative medical system is respectively 70%, 65% and 10%, but the new rural cooperative medical system~ open rate is lower. 13 community hospitals adopt the two - line management mode of revenue and expenditure. The implementation rate of national essential drugs of service centers and health service stations is respectively 100% and 84.6%, and the former＇s is higher than the latter＇s （P 〈 0.05 ）. The implementation rate of national essential drugs, provincial supplemented directory and municipal supplemented directory is 75% , 80% and 40%, and the former two＇sis higher than the last one＇s（P 〈0.05）. 17 Community hospitals carry out the pharmaceutical retail sales price, whose implementation rate is 85%. Conelusion： There are ascension space for community hospitals for the implementation of health policy. Improving the service quality should focus on the rural cooperative medical care work＇s implementation.
