摘要
目的通过调查医院压疮现患率、发生率及压疮预防工具的使用情况,分析导致压疮发生的医院相关危险因素,探讨医院压疮管理对策。方法采用压疮现患率调查表,分别于2013年7、10月及2014年1、4月某日(8:00-18:00)对青岛大学附属医院的住院患者进行调查,严格按照护理部制定的统一标准填写调查表及统计数据。结果 4次调查共纳入住院患者11 624例,实查率为98.59%;医院压疮现患率为1.46%,发生率为0.59%;压疮首发部位为骶尾部与足跟部,急诊科与重症医学科为压疮现患率高发科室;Ⅰ期压疮患者预防性措施(压疮敷料、减压气垫、翻身措施)的落实率分别为32.5%、42.5%、82.5%,Ⅱ期压疮患者预防性措施的落实率分别为79.3%、79.3%、93.5%。结论该院在压疮管理与预防方面还存在诸多问题,尚需进一步改善与加强压疮监管及预防力度,早期合理采取预防性措施,对压疮现患率较高的科室采取重点干预,及时发现日常监测工作中的疏漏。
Objective To analyze the prevalence and incidence rate of pressure ulcers and the using of preventing tools,analyze the hospital related risk factors and provide related reference for clinical nursing management.Methods The hospitalized patients participated in the study and were investigated by using questionnaires of prevalence rate of pressure ulcer on someday （8：00-18：00）July and October 2013,January and April 2014 respectively.The results were analyzed by unified tools,ways and judgmental criteria.Results 11624 cases of hospitalized patients were investigated by four times,and the actual check rate was 98.59%.The prevalence rate was 1.46%,and incidence rate was 0.59%.Four times survey showed that the sacrococcygeal region and foot heel were the frequent positions and prevalence rate of pressure ulcer were mostly happened at the emergency department and ICU.Patients with stage Ⅰ of preventive measures （dressings,vacuum pad,roll over）utilization rate were 32.5%、42.5% and 82.5% respectively,Patients with stage Ⅱ preventive measures （dressings,vacuum pad,roll over）utilization rate were 79.3%、79.3%and 93.5% respectively.Conclusion There are lots of problem existing in this hospital about pressure ulcer management and prevention,so it is required further improvement on pressure ulcer monitoring and prevention,and we should take measures to reasonably allocate and use the resource and take targete d intervention on key departments to timely find out the omission on the daily work.
