摘要
目的对经外周静脉置入中心静脉导管(PICC)置管的感染因素进行分析,并探讨预防护理措施,以更好地指导PICC置管。方法选择医院2011年1月-2013年7月PICC置管患者126例为研究对象,对其临床资料进行回顾性分析,观察患者PICC置管的感染情况,并对感染的相关因素进行统计分析;使用SPSS 19.0软件对数据进行统计分析。结果 126例PICC置管患者中,9例患者发生不同程度的导管相关性感染,感染率为7.14%;共分离出病原菌9株,其中凝固酶阴性葡萄球菌4株、金黄色葡萄球菌3株、阴沟肠杆菌及大肠埃希菌各1株;穿刺次数、护理人员操作经验、刺激性药物使用、白细胞数偏低、静脉炎以及导管留置时间与PICC置管感染具有相关性(P<0.05)。结论引发PICC置管感染的因素较多,导致PICC置管后导管相关性感染的出现率居高不下,掌握PICC置管患者感染因素并做好相应的预防护理措施对于降低感染率有着重要的意义。
OBJECTIVE To investigate the related factors for the peripherally inserted central venous catheter （PICC）‐associated infections and explore the preventive nursing measures so as to guide the placement of PICC . METHODS A total of 126 patients who underwent the PICC placement from Jan 2011 to Jul 2013 were recruited as the study objects ,then the clinical data of the participants were retrospectively analyzed ,the prevalence of the PICC‐associated infections was observed ,the related factors for the infections were statistically analyzed ,and the statistical analysis was performed with the use of SPSS19 .0 software .RESULTS The catheter‐associated infections occurred in 9 of 126 patients undergoing the PICC catheterization ,with the infection rate of 7 .14% .Totally 9 strains of pathogens have been isolated ,including 4 strains of coagulase‐negative Staphylococcus ,3 strains of Staphylococcus aureus ,1 strain of Enterobacter cloacae ,and 1 strain of Escherichia coli .The prevalence of the PICC‐associated infections was related to the frequency of puncture ,experience of operation of the nursing staff , use of stimulant drugs ,low white blood cell counts ,phlebitis ,and catheter indwelling time （P〈 0 .05） . CONCLUSION There are a variety of factors for the PICC‐associated infections ,which lead to the high prevalence rate of the catheter‐associated infections after the PICC catheterization .It is of great significance to master the re‐lated factors for the infections in the patients undergoing PICC catheterization and take appropriate nursing meas‐ures so as to reduce the infection rate .
