摘要
目的:研究外周敏化与腹泻型肠易激综合征(IBS-D)中医证候的关系,初步探讨"肝主疏泄助脾胃运化"的生物学内涵。方法:进行IBS-D中医证候流行病学调查,检测外周敏化机制的核心指标;运用SPSS 19.0软件进行独立样本的t检验、多因素的方差分析、非参数检验、相关分析等统计分析。结果:IBS-D的主要证候是肝郁脾虚证,占44.5%、脾胃虚弱证,占20.5%、寒湿困脾证,占11.4%。5-羟色胺(5-HT)与肝郁气滞证、脾肾阳虚证的相关系数分别是0.130和0.197;降钙素相关基因肽(CGRP)与肝郁脾虚证和肝郁气滞证的相关系数分别是0.108、0.116。结论:IBS-D的外周敏化对IBS-D中医证候的分布有重要影响,CGRP→5-HT/5-HT3R信号转导通路的调控可能是其生物学机制。
Objective: To study the relationship between TCM syndrome of IBS-D and peripheral sensitization, so as to research the biological connotation of liver governing regulation to help spleen-stomach transportation and transformation primarily. Methods: Epidemiological investigation of TCM syndromes of IBS was performed, and the core indexes of peripheral sensitization were detected. SPSS 19.0 was used to finish T-test for independent samples, multivariate analysis of variance, non- parametric tests and statistical analysis of correlation analysis. Results: The main TCM syndromes of IBS-D were syndrome of liver depression and spleen deficiency (44.5%), spleen-stomach weakness syndrome (20.5%), cold-dampness disturbing spleen syndrome (11.4%). The correlation coefficient between 5-HT and liver qi depression syndrome, spleen-kidney yang deficiency syndrome was 0.130 and 0.197, respectively. The correlation coefficient between CGRP and liver depression and spleen deficiency syndrome, liver qi depression syndrome was 0.108 and 0.116, respectively. Conclusion: Peripheral sensitization of IBS-D has an important influence for TCM syndrome of IBS-D, and the regulation of signal transduction pathways (CGRP---,5-HT/5-HT3R) may be the biological mechanism.
