炎症标志物血清降钙素原和超敏C反应蛋白与多囊卵巢综合征相关因素研究 被引量:9

Correlation between inflammatory markers of serum procalcitonin & hs-CRP and related indicators of polycystic ovary syndrome

摘要 目的通过研究多囊卵巢综合征(PCOS)患者炎症标志物血清降钙素原和超敏C反应蛋白(hs-CRP)的水平,探讨炎症标志物血清降钙素原、hs-CRP与PCOS的有关指标相关性。方法选择40例PCOS患者为病例组,同期来院就诊的40例不孕不育症患者为对照组。并根据体重指数(BMI)水平分为BMI≥25kg/m2的PCOS患者为PCOS肥胖组(A组,20例),BMI<25kg/m2的PCOS患者为PCOS非肥胖组(B组,20例);BMI≥25kg/m2的不孕不育患者为非PCOS肥胖组(C组,20例),BMI<25kg/m2的不孕不育患者为非PCOS非肥胖组(D组,20例)。四组研究对象入院后采集静脉血,进行血清降钙素原、hs-CRP、空腹胰岛素(FIN)、血糖、性激素、性激素结合蛋白(SHBG)的检测;PCOS组与对照组均数间比较及PCOS组胖瘦组之间比较采用两独立样本t检验,各因素间关系采用Spearman相关性分析。结果血清降钙素原与PCOS患者以下指标如BMI、腰臀比(WHR)、稳态胰岛素评价指数(HOMA-IR)、低密度脂蛋白胆固醇(LDL-C)、FIN、卵泡刺激素(FSH)、黄体生成素(LH)、睾酮(T)、卵泡(Fc)总数、左卵巢Fc个数、右卵巢Fc个数、SHBG等均有相关性(P<0.05)。hs-CRP与PCOS患者以下指标:BMI、WHR、HOMA-IR,FIN、FSH、LH、T、雌二醇(E2)、SHBG等均有相关性(P<0.05)。结论炎症标志物血清降钙素原、hs-CRP与PCOS具有相关性,但炎症介质在PCOS中的作用机制尚需研究,以期阐明其相关机制。 Objective: To explore the correlation between the inflammatory markers of procalcitonin hypersensitive C-reactive protein(hs-CRP)and related indicators of polycystic ovary syndrome(PCOS).Methods:Forty patients with PCOS were selected in the observe group,and 40 infertile patients in the control group.The patients were subdivided to 4groups according to BMI levels:the obese(BMI≥25kg/m^2)patients with PCOS in group A(n=20),the non-obese(BMI〈25kg/m^2)patients with PCOS in group B(n=20),the obese(BMI≥25kg/m^2)patients without PCOS in group C(n=20),and the non-obese(BMI25kg/m^2)patients without PCOS in group D(n=20).The serum samples were collected from the patients of the four groups for detecting procalcitonin,hs-CRP,fasting insulin(FIN)and blood glucose,hormone levels and sex hormone binding globulin(SHBG).The parameter were analyzed and compared among the four groups by using two independent samples t-test and Spearman correlation analysis.Results:In the patients with PCOS,the serum calcitonin were correlated with BMI,waist-to-hip ratio(WHR),Homeostasis model assessment for insulin resistance(HOMA-IR)LDL-C,FIN,FSH,LH,T,E2,total number of follicles,number of follicles in left or right ovary,SHBG(P〈0.05).The hs-CRP were correlated with BMI,WHR,HOMA-IR,FIN,FSH,LH,T,E2,SHBG(P〈0.05).Conclusions: Serum inflammatory markers procalcitonin hs-CRP are correlated with some parameter of the patients with PCOS,but the mechanism for inflammatory mediators in the pathogenesis of PCOS is not clear,which need further investigation
