摘要
目的观察自拟养阴清热、活血利水中药组、环磷酰胺组、中药组方联合环磷酰胺对狼疮性肾炎患者血清IFN-γ、IL-4、TGF-β1表达的影响。方法收集狼疮性肾炎病例共95例,随机分为中医组、西医组和中西医结合组,西医组31例,中医组31例,中西医结合组33例。中医组服中药组方每日1剂,分2次口服;西医组静滴环磷酰胺1.0 g/月;中西医结合组在中药组方的基础上静滴环磷酰胺0.6 g/月。分别观察3组治疗前和治疗6月后外周血清IFN-γ、IL-4、TGF-β1的表达。结果治疗前后中医组、西医组IFN-γ、IL-4、TGF-β1的组内比较(P<0.05),中西医组IFN-γ、IL-4、TGF-β1的组内比较(P<0.01)。治疗后IL-4 3个组的组间比较结果示中西医组在降低IL-4的水平方面分别优于中医组和西医组(P<0.05)。结论养阴清热、活血利水中药组方可能是通过改善狼疮性肾炎患者血清中干扰素-γ、IL-4及TGF-β1的表达,从而发挥治疗效果。
Objective To observe serum IFN-γ,IL-4,TGF-β1 in lupus nephritis patients treated with decoction of nourishing yin and clearing away heat,activating blood and promoting diuresis,cyclophosphamide or cyclophosp hamide plus the above decoction in accordance with syndrome differentiation. Methods We collected 95 lupus nephritis cases from rheumatology department and outpatient of shanghai municipal TCM hospital affliated to shanghai TCM University. They were divided into chinese medicine group（ 31 cases）,western medicine group（ 31 cases） and integrative medicine group（ 33 cases） by random assignment. Chinese medicine group： traditionnal chinese medicine decoction taken twice a day; Western medicine group： intravenous cyclophosphamide 1. 0 g / month; Integrative medicine group： chinese medicine decoction plus intravenous cyclophosphamide 0. 6 g / month. All the patients were treated6 months respectively. Serum IFN-γ,IL-4,TGF-β1 before and after treatment in the three groups was observed. Results Both Chinese and western medicine groups improved IFN-γ,IL-4,TGF-β1 after treatment（ P〈0. 05）. Integrated medicine group improved IFN-γ,IL-4,TGF-β1 after treatment more（ P〈0. 01）. The analysis of three groups after treatment showed the integrated medicine group decreased IL-4 better than the other two（ P〈0. 05）. Conclusion Herbal formula with the function of nourishing yin and clearing away heat,activating blood and promoting diuresis may improve the expression of serum IFN-γ,IL-4,TGF-β1 in LN patients to exert therapeutic effect.
