c-met和EGFR蛋白表达与声门型喉磷癌术后复发的关系

Relationship between c-met and EGFR expression and postoperative recurrence in glottic laryngeal squamous cell carcinoma

摘要 目的探讨表皮生长因子受体(epidermal growth factor receptor,EGFR)和肝细胞生长因子受体(mesenchymal-epithelial transition factor,c-Met)在声门型喉鳞癌中的表达及与术后复发之间的关系。方法共纳入66例声门型喉磷癌患者,复发组和未复发组各33例。免疫组化法检测c-Met及EGFR在原发灶组织的表达情况,用H评分法对免疫组化结果进行评估。定义H评分值高于中位值为高表达,相反为低表达。结果在总群体中,c-Met和EGFR的H评分中位值分别为150(0~270)和225(0~270)。单变量Logistic回归分析表明,c-Met或EGFR高表达患者的复发风险显著高于低表达患者(OR=7.111,P〈0.001;OR=5.290,P=0.002)。逐步回归分析表明c-Met或EGFR的高表达均可作为声门型喉癌术后复发的独立危险因素(OR=5.909,P=0.002;OR=4.231,P=0.013)。ROC曲线分析显示联合检测c-Met和EGFR的表达情况对评估术后复发风险具有良好的预测效能(AUC=0.795,95%CI:0.685~0.905)。结论 c-Met和EGFR的表达均与声门型喉磷癌术后复发密切相关,可作为术后复发的有效预测指标。 Object ve To explore the relationship between mesenchymal-epithelial transition factor (c-Met) and epidermal growth factor receptor (EGFR) expression and postoperative recurrence in glottic laryngeal squamous cell carcinoma (LSCC). Methods Immunohistochemistry accompanied with H scores was utilized to evaluate the expression of c-Met and EGFR in primary focus tissues of 66 patients with glottic LSCC, who were equally divided into a recurrence group and a non-recurrence group. If H scores were greater than the median values, the c-met and EGFR expression was defined as high expression, and conversely, low expression was defined. Results The median values of H-scores of c-Met and EGFR were 150 (0 -270) and 225 (0 -270), respectively. The univariate logistic regression analysis showed that the recurrence risk was significantly increased in the patients with high expression of c-Met or EGFR compared with the patients with low expression ( OR = 7. 111, P 〈 0. 001 ; OR = 5. 290, P = 0. 002). Stepwise regression analysis suggested that high expression of c-Met or EGFR was independent prognostic factor for recurrence after LSCC operation ( OR = 5. 909, P = 0. 002 ; OR = 4. 231, P = 0. 013 ). ROC curve analysis showed that combined detection of c-Met and EGFR expression had moderate predictive value for postoperative recurrence (AUC = 0. 795, 95% CI: 0. 685 - 0. 905 ). Conclusion The expression levels of c-Met and EGFR are closely correlated with postoperative recurrence of glottic LSCC, and can be used as indicators for recurrence of glottic LSCC.
