摘要
目的对不同温度、时间制备的新鲜血浆进行检测,观察FⅤ、FⅦ、FⅧ、FⅨ、FⅩ凝血因子活性的变化情况,以发挥对临床血浆输注或制备冷沉淀选择原料血浆的指导作用。方法在2℃~6℃、15℃左右和室温各制备20份血浆,每一试管留取5ml,同一温度、不同时间来源于同一份血浆,分别于6h、8h、10h、14h、18h速冻标本,并在72h内将速冻成固体的血浆与37℃水温解冻后立即运用血凝法检测各凝血因子活性。结果 2℃~6℃环境中保存的血浆在6h、8h、10h、14h、18h内Fib、FⅦ、FⅨ、FⅩ因子活性的降低不是太明显,但均在正常范围内,而FⅤ、FⅧ因子活性在6~10h也在正常范围,但在14~18h时出现低于正常值的现象。15℃左右和室温保存的血液在6~10h制备FFP中的Fib、FⅦ、FⅨ、FⅩ因子活性下降速度不明显,FⅤ、FⅧ因子随着温度升高和保存时间的延长活性下降速度明显,8~10h与6h相比,所有凝血因子水平的降低都有统计学意义。结论采集的血液保存在2℃~6℃环境中制备的新鲜冰冻血浆中凝血因子活性优于15℃、室温情况下保存血液制备的FFP,建议血站成分科制备冷沉淀原料血浆时,最好用2℃~6℃冷藏环境6~10h内和15℃、室温环境下8h内制备的新鲜冰冻血浆作为制备冷沉淀的原料血浆。
Objective Five blood coagulation factors of Fib,FⅤ,FⅦ,FⅧ,FⅨ and FⅩ were tested in fresh plasma prepared at different temperatures and times in order to guide the preparation of plasma and cryoprecipitation for clinical transfusion.Methods Twenty samples of plasma were prepared under 2℃~6℃,15℃ and room temperature,respectively,with 5ml in each tube.All test samples were frozen for 6h,8h,10 h,14h,and 18 h,and thawed at 37℃ water bath.The activity of coagulation factors of the plasma was tested with clotting method in 72 h.Results No activity of the 5factors was reduced preserved under 2℃~6℃for 6h,8h,10 h,14h,and 18 h,respectively.Factors FⅤ and FⅧ,however,partially lost the activity under 14h^18h.No remarkable reduction of activity of Fib,FⅦ,FⅨ,and FⅩ was found preserved at 15℃ or room temperature for 6h^10h,but FⅤ and FⅧ gradually lost their functions with increase of temperature and the preservation duration.Conclusion Storage of fresh frozen plasma at 2℃~6℃ might keep the activity of coagulation factors compared with stored at 15℃ or at room temperature.Plasma for cryoprecipitation should be prepared at 2℃~6℃ within 6~10h,or at 15℃ or room temperature within 8h.
