摘要 目的观察吞咽功能训练结合神经肌肉电刺激治疗脑卒中后神经源性吞咽功能障碍的疗效。方法临床纳入脑卒中后神经源性吞咽功能障碍患者76例,根据入院后治疗方案的不同分为研究组与对照组。研究组给予吞咽功能训练结合神经肌肉电刺激治疗,对照组仅给予神经肌肉电刺激治疗。观察察两组患者治愈率、吞咽造影检查(VFSS)、洼田氏饮水试验,并进行心理状态量评分调查。结果观察组临床治愈率68.42%,显著高于对照组的42.11%(P<0.05)。两组患者治疗后洼田氏饮水试验的分级较治疗前均有一定程度的好转,且研究组改善程度优于对照组(P<0.05)。研究组治疗后VFSS评分明显高于对照组(P<0.05)。观察组患者焦虑自评量表(SAS)、抑郁自评量表(SDS)、自尊量表(SES)平均得分均显著低于对照组,差异有显著性(P<0.05)。结论吞咽功能训练结合神经肌肉电刺激治疗脑卒中后神经源性吞咽功能障碍,具有较好的临床疗效,能够显著改善患者的吞咽障碍,同时可以明显缓解患者的负面情绪。 Objective To observe the treatment effect of swallowing function training combined with neuromuscular electrical stimulation to the neurogenic swallowing dysfunction after stroke. Methods Seventy-six patients who occurred neurogenic swallowing dysfunction in clinical were selected. According to the treatment after admission,they were divided into the study group and control group. The research group were treated by swallowing function training combined with neuromuscular electrical stimulation therapy,while the control group were only given the neuromuscular electrical stimulation therapy. The patients cure rate,swallowing angiography( VFSS),Watian drinking water test and mental status score of investigation were observed. Results The clinical cure rate of research group was 68. 42% and the clinical cure rate of the control group was 42. 11%. The difference was significant( P 0. 05). After treatment before drinking Tian grade depression treatment trials have a certain degree of improvement. The degree of improvement in the study group was better than the control group( P 0. 05). VFSS scores in the study group after treatment was significantly higher than the control group( P 0. 05). SAS,SDS,SES average scores in study group were significantly lower than the control group. The difference were significant( P 0. 05). Conclusion The swallowing function training combined with neuromuscular electrical stimulation of nerves can treat neurogenic swallowing dysfunction after stroke. This method have good clinical curative effects,which can significantly improve dysphagia and negative emotions of patients.
