摘要
随着生活水平的提高及生活习惯、饮食结构的改变,脂肪肝的患病率逐年升高。近年来,中医药在脂肪肝的证候规律、辨证施治等方面取得了一定进展,尤其是从痰论治方面进行了不少临床、实验研究,故探讨和整理从痰论治脂肪肝的机制。首先就历代医家从痰论治脂肪肝的理论进行了梳理,其次从饮食不节、损伤脾胃、脾失健运生痰,情志不畅、肝失条达、水血郁滞成痰,劳逸失度、脾肾受损、水液不化成痰等3个方面阐述了脂肪肝痰证的形成机制。最后,结合脂肪肝的西医学发病机制,总结归纳了从痰治疗脂肪肝的机理。研究表明,从痰论治脂肪肝的机制在于调节脂质代谢紊乱、改善胰岛素抵抗、减轻氧化应激与脂质过氧化、提高肝细胞的分裂增殖及减轻炎症反应等多个方面。
With the improvement of living standards and the change of lifestyle and diet structure, the prevalence of fatty liver increased. In recent years, traditional chinese medicine made progresses in the law of syndrome and the syndrome differentiation treatment of the fatty liver. Especially, many clinical and experimental researches were performed in the aspects of Phlegm treatment. We aimed to sort out the mechanism of fatty liver treatment from Phlegm. Firstly, we combed the theory of ancient physicians treating fatty liver from Phlegm. Secondly, we explained the formation mechanism about Phlegm syndrome of fatty liver in three aspects： improper diet and injury of spleen and stomach leading to the loss of transport of spleen function and produce sputum, poor emotional ease inducing the failure of liver transport and catharsis and subsequently retention and blood stasis resulting sputum. , damage of the spleen and stomach by overworking stopping fusion liquid water form sputum. Finally, we summarized the mechanisms of fatty liver treatment from Sputum combining with the pathogenesis of fatty liver in modern medicine. Including of regulation of lipid metabolism disorders, improving insulin resistance, reducing oxidative stress and lipid peroxidation, increasing the proliferation of liver cells and inhibiting inflammation reaction.
