摘要
目的:分析雄激素联合免疫抑制剂治疗再生障碍性贫血患者的综合应用价值。方法:回顾性分析基础治疗(对照组)、在基础治疗前提下加载抗淋巴细胞球蛋白(ALG,观察组)治疗再生障碍性贫血的疗效,对照组和观察组分别为80例。治疗前后对两组患者血红蛋白(Hb)、白细胞(WBC)、红细胞(RBC)、血小板(PLT)计数,血清炎症因子白细胞介素2、6、17、(IL-2、IL-6、IL-17),肿瘤坏死因子-α(TNF-α)、干扰素-γ(IFN-γ)水平以及疾病相关指标转化生长因子-β1(TGF-β1)、促红细胞生成素(EPO)、促血小板生成素(TPO)、可溶性血管细胞黏附分子-1(sVCAM-1)、血管内皮生长因子(VEGF)水平进行检测,比较组间差异。结果:治疗后第3、6个月,观察组患者Hb、WBC、RBC、PLT计数均显著高于对照组;血清炎症因子IL-2、IL-6、IL-17、TNF-α、IFN-γ水平显著低于对照组;血清TGF-β1、VEGF水平显著高于对照组,而EPO、TPO、sVCAM-1则显著低于对照组,差异均有统计学意义(P<0.05)。结论:ALG联合环孢霉素、雄激素治疗再生障碍性贫血能有效抑制免疫反应,增加骨髓细胞的造血功能,在再生障碍性贫血的治疗中具有较高的应用价值。
Objective： To observe effect of androgen combined with immunosuppressants on patients with aplastic anemia（AA）. Methods：All patients were divided into basic treatment（control group） and basic treatment combined with antilymphocyte globulin （ALG）（observation group）, with 80 cases in each group. Indexes of complete blood count of Hb, WBC, RBC, PLT, serum levels of IL-2, IL-6, IL-17, TNF-a, IFN-γ and levels of disease-related indicators TGF-β1, EPO, TPO, sVCAM-1, VEGF of two groups before and after treatment were detected and compared. Results=After 3, 6 months of treatment, the complete blood count of Hb, WBC, RBC, PLT of observation group were significantly higher than control group, the serum levels of IL-2, IL-6, IL-17, TNF-α IFN-γ were all lower than control group, and the serum levels of TGF-β1, VEGF were higher than control group, while and the serum levels of EPO,TPO, sVCAM-1 were significantly lower than control group （all P〈0.05）. Conclusions=ALG combined cyclosporine, androgens can effeetively suppress immune response, improve the hematopoietic function of bone marrow cells for patients with AA, and is highly valuable in the treatment of AA.
