再生障碍性贫血的中医气化论治策略 被引量:6

Treatment strategy of aplastic anemia based on TCM qi transformation theory

摘要 再生障碍性贫血之病因病机尚需进一步阐明,其治疗经历了以补脾益气到补肾为本的过程。目前临床一般遵从脏腑辨证的规律,从肾、脾入手进行论治。文章从气化理论的角度分析再障病情的发展变化,提出再障的病机以肾为本,以脾为标,以心为主,以肝、肺为变的总原则。胃为生气之源,胆为升发之本,胆随胃降,胃随胆升,对于再障的辨治至关重要,符合气化整体观念之临床真实世界。 The etiology and pathogenesis of aplastic anemia need to be further clarified,and its treatment process has experienced with invigorating spleen and replenishing qi to tonifying kidney.At present,the clinical generally follow the rules of syndrome differentiation of zang-fu viscera,and treatment from the kidney and spleen.In this article,the development and changes of aplastic anemia were analyzed from perspective of qi transformation theory.The pathogenesis of aplastic anemia was put forward.And its general principles are that tonifying kidney as radical treatment,invigorating spleen as symptomatic treatment,invigorating heart as main treatment and regulating liver and lung as modified treatment.That stomach being source of qi,gallbladder being root of ascending and dredging,gallbladder qi with stomach qi descending and stomach qi with gallbladder qi ascending,are essential for the syndrome differentiation and treatment of aplastic anemia,and it is consistent with the overall concept of qi transformation in clinical real-world.
