摘要
目的 探讨人脑星形细胞瘤瘤周水肿产生的机制 ,指导临床医生对患者病情判断、预后估计和治疗。方法 透射电镜观察星形细胞瘤微血管超微结构改变 ;免疫组织化学方法检测 33例人脑星形细胞瘤血管内皮生长因子 (vascularendothelialgrowthfactor,VEGF)表达 ,并研究其与肿瘤病理分级和瘤周水肿 (peritumoralbrainedema ,PTBE)的关系。结果 星形细胞瘤微血管超微结构表现为 :微血管形态不规则 ,结构不完整 ,内皮细胞缺氧退变 ,毛细血管通透性增强。星形细胞瘤VEGF表达与肿瘤病理分级和PTBE明显相关 (P <0 0 0 2和P <0 0 2 )。结论 人脑星形细胞瘤瘤区内微血管超微结构的特征性改变是PTBE产生的形态学基础。星形细胞瘤VEGF表达与肿瘤病理分级和PTBE明显相关。肿瘤细胞分泌VEGF是星形细胞瘤PTBE产生的重要原因。临床上可把PTBE和VEGF表达程度作为参考因素进行星形细胞瘤分级 。
Objective To explore possible mechanism in the production of PTBE of gliomas and to guide the patient clinical evaluation,prognosis estimating and trearment. Methods Transmission electronic microscope(TEM) was used to observe the microvascular ultrastructure within astrocytomas.SP immunohistochemical technique was used to detect the expression of VEGF in 33 astrocytomas which was correlated to the grading of astrocytomas and PTBE. Results The tumoral microvascular ultrastructure under TEM showed that the microvascular morphosis was irregular;their structure was not intact;the endothelial cells of the changes of hypoxia and capillary permeability increased.There were obviously significant correlations between expression of VEGF,grading and PTBE in astrocytomas( P< 0 002 and P< 0 02). Conclusion The characteristic changes of microvascular ultrastructure in human astrocytomas are the morphological base of the production of PTBE.The expression of VEGF correlates well with astrocytomas pathologic grading and PTBE.The secretion of VEGF from astrocytoma cells is an important cause to the production of PTBE.Therefore,the degree of PTBE and expression of VEGF can be served as the preference factors to evaluate astrocytoma's grading and to analyze patients' prognosis.
