摘要
胃癌前病变是近年来胃癌防治学研究的重点 ,笔者将近几年中医药的研究成果作一综述。其常见病因有饮食不节 ,情志失调 ,劳倦内伤 ,用药不当 ,久病体虚等。关于病机较为统一的认识为 :病位在脾胃 ,与肝密切相关 ,病理性质总属本虚标实。临床主要研究方法 :①辨证分型论治 ;②基础方加减论治 ;③固定专方治疗。存在问题 :①诊断标准不统一 ;②科研设计不完善 ;③疗效标准不规范。今后努力方向 :充分利用生命科学中的高新技术 ,从分子、基因、细胞凋亡角度深入研究 。
The results of the investigations made by the authors show that the common causes of the pathological changes before gastric carcinoma include improper food intake, emotional disturbance, fatigue, internal damage, improper drug administration, weak body physique due to long lasting diseases, etc. As far as the pathogenesis is concerned, the authors believe that the disease is located in the spleen and stomach and is closely related to the liver. The pathological nature of the disease is deficiency of the constitution and excess of the symptoms. The main clinical research methods include: (1) giving treatment based on the differentiation of syndromes; (2) using basic prescriptions with addition or removal of certain ingredients; and (3) using fixed prescriptions in treatment. The problems remained include: (1) inconsistency in diagnostic criteria; (2) imperfect research design; and (3) lack of standards of therapeutic effect. The authors will devote their effort to making full use of the high and new technology in life science to make further investigations into molecule, gene and cell apoptosis so as to reveal the mechanism of reversion of the pathological changes before gastric carcinoma.
