CEA与CYFRA21-1在非小细胞肺癌血清和胸腔积液差异表达及其相关性分析 被引量:18

摘要 目的:比较肿瘤标记分子CEA与CYFRA21-1在非小细胞肺癌(NSCLC)血清和胸腔积液表达差异及其相关性。方法:检测62例并发恶性胸腔积液的NSCLC患者血清和胸腔积液中CEA与CYFRA21-1水平,以28例并发良性胸腔积液的感染性肺部疾病患者作为对照,比较两类肿瘤标记分子在NSCLC血清和恶性胸腔积液的差异性表达,并分别分析CEA和CYFRA21-1水平在NSCLC两类体液之间的相关性。结果:CEA与CYFRA21-1在NSCLC血清和恶性胸腔积液表达水平均差异具有显著性(P<0.000 1),两者在血清诊断实验中的曲线下面积(AUC)分别为0.9和0.998;在胸腔积液鉴别诊断中的AUC值则分别为0.896和0.765。相关性分析表明CEA和CYFRA21-1在NSCLC血清和胸腔积液之间相关系数分别为0.684(P<0.01)和0.244(P=0.056)。结论:CEA与CYFRA21-1含量在NSCLC血清与恶性胸腔积液之间差异均具有显著性,CEA表达水平在血清和胸腔积液之间具有显著相关性,而CYFRA21-1则呈低度相关。 Objective To investigate the differential expression and correlation of CEA and CYFRA21-1 in pleura] effusions (PEs) and serum from patients with NSCLC. Methods Based on quantification of CEA and CYFRA21-1 in PEs and sera from 62 patients with NSCLC and 28 patients with inflammatory pulmonary diseases, the differential expression and correlation of the two biomarkers in serum and PE of patients with NSCLC were evaluated respectively. Results Significantly different expression of CEA and CYFRA21-1 were observed between serum and PE from patients with NSCLC (P 〈 0.000 1). The area under the curve (AUC) of CEA and CYFRA21-1 in serological diagnosis of patients with NSCLC was 0.9 and 0.998, whereas AUC of the two biomarkers in PE identification of patients with NSCLC was 0.896 and 0.765, respectively. Correlation analysis showed that Pearson correlation coefficient of CEA and CYFRA21-1 level between PEs and sera from patients with NSCLC was 0.684 (P 〈 0.01 ) and 0.244 (P = 0.056), respectively. Conclusions Both CEA and CYFRA21-1 have significant differentiation between serum and PE from patients with NSCLC. However, a moderate correlation for CEA and a weak correlation for CYFRA21-1 exist between PE and serum from patients with NSCLC.
