摘要
目的:探讨益气养阴汤对老年非小细胞肺癌术后化疗发热患者免疫功能的影响。方法:选取60例非小细胞肺癌术后化疗发热的老年患者,随机分成两组,每组30例。对照组给予吉西他滨联合顺铂方案治疗,研究组在此基础上再予益气养阴汤治疗,检测治疗前后外周血T淋巴细胞亚群及免疫球蛋白水平,测定肿瘤标志物及评估生活质量,密切观测体温及降温时间,比较近期疗效及中医证候疗效。结果:与治疗前比较,对照组CD_4^+、自然杀伤细胞(natural killer cell,NK)降低,CD_8^+升高(P<0.05),研究组CD_8^+降低,CD_3^+、CD_4^+、NK升高(P<0.05),两组比较,差异有统计学意义(P<0.05);对照组免疫球蛋白G(immunoglobulin,Ig G)、Ig M及Ig A水平降低,研究组Ig G、Ig M及Ig A水平升高,两组比较,差异有统计学意义(P<0.05);研究组癌胚抗原(carcino-embryonic antigen,CEA)及CA153水平、生命质量测定量表(quality of life questionare-core 30,QLQ-C30)评分均降低,卡氏评分升高,与对照组比较,差异有统计学意义(P<0.05);研究组治疗后12 h体温明显降低,降温时间明显较对照组短(P<0.05),且近期疗效以及中医证候疗效较对照组显著(P<0.05)。结论:益气养阴汤可显著改善老年非小细胞肺癌患者术后化疗所致发热症状,显著改善免疫功能,延缓病情进展并提高生活质量。
Objective：To explore the effect of Yiqi Yangyin Decoction on the immune function in elderly patients with Non-small Cell lung cancer in postoperative chemotherapy fever.Methods：60 elderly patients with postoperative chemotherapy fever of Nonsmall cell lung cancer were selected and randomly divided into 2 groups,with 30 cases in each group.The Control group received chemotherapy of Gemcitabine and Cisplatin,while the study group received Yiqi Yangyin Decoction in addition to that of the control group.Peripheral blood T lymphocyte subsets and immunoglobulin levels,tumor markers,patients＇quality of life,body temperature and cooling time,recent efficacy and TCM symptoms of both groups before and after treatment were compared.Results：Com- and NK increased in the study group（P〈0.05）.The levels of IgG,Ig M and IgA in the control group decreased,while those in the study group increased in the control group（P〈0.05）.The differences between the both groups were statistically significant（P〈0.05）.Compared with the control group,the scores of CEA and CA153 and QLQ-C30 in the study group were significantly lower than those in the control group（P〈0.05）.The temperature and cooling time of the study group were significantly higher than those of the control group（P〈0.05）,and the short-term efficacy and TCM syndrome were higher than those in the control group（P〈0.05）.Conclusion：Yiqi Yangyin Decoction was effective in treatment of postoperative chemotherapy fever for elderly patients with Non-small cell lung cancer,and it can significantly improve patients＇immune function and quality of life.
