摘要
目的探讨社区高血压患者治疗依从性和社会功能的相关影响因素,以提高社区高血压的管理效果。方法 2011年9月—2012年12月通过简单随机抽样法抽查上海市两个社区内高血压患者,分别进入干预组(300例)和对照组(300例),干预组和对照组分别采用"4CH8"社区健康管理模式和原管理模式。采用自制问卷调查两组患者的各观察指标,分析其治疗依从性和社会功能的影响因素。结果所有调查对象中67.1%(369例)治疗依从性好,69.5%(382例)社会功能好。管理组别、体质指数、血脂和知晓程度得分是高血压患者治疗依从性的影响因素,而年龄、婚姻状况、管理组别、并发症和患病时间是社会功能的影响因素,其中管理组别是其共同的保护因素(OR=7.338和10.124)。结论社区高血压患者治疗依从性和社会功能受不同因素的影响,"4CH8"社区健康管理模式对两者均是保护因素,今后可采取综合的管理措施对社区高血压患者进行全方位管理。
Objective To investigate the main influential factors of treatment compliance and social functions in hyper- tensive patients in community. Methods Totally 600 patients with hypertension were recruited from two communities in Shanghai through simple random sampling from September 2011 to December 2012. They were divided into intervention group （ n = 300） and control group （n = 300）. The intervention group applied ＂ 4CH8＂ model of community health management, while the con- trol group applied the original service model. A survey was carried out among the subjects using a self - designed questionnaire. Results 67.1% （369/550） had good treament compliance, while 69. 5% （382/550） had good social functions. Manage- ment model, BMI, blood lipid level and disease awareness were the influential factors of the patients＇ treatment compliance; while age, marital status, management model, comphcations and the duration of the disease were the influential factors of social function. Management model was the common protective factors, of which ＂ dCHS＂ model could improve treatment compliance and social functions （ OR = 7. 338 and 10. 124）. Conclusion Many different factors can effect treatment compliance and related social functions in hypertensive patients. ＂ 4CH8＂ model of community chronic disease management should be taken as the man- agement measures for patients with hypertension in community.
