臂踝脉搏波传导速度与冠状动脉病变程度的相关性研究 被引量:3

Correlation Research on the Brachial Ankle Pulse Wave Velocity with the Severity of Coronary Artery Disease

摘要 目的:探讨臂踝脉搏波传导速度(brachio-ankle pulse wave velocity,baPWV)与冠状动脉粥样硬化狭窄程度的相关性。方法:选择198例行冠脉造影的患者为研究对象,根据冠状动脉管腔狭窄程度分为非冠心病组(对照组,50例)和冠心病组(148例);并将冠心病组分为单支病变组(48例)、双支病变组(56例)及多支病变组(44例),所有患者均采用动脉硬化检测仪检测baPWV,观察各组baPWV值及Gensini积分的变化,并对其行相关性分析。结果:CAD组多支病变、双支病变、单支病变亚组及对照组冠状动脉Gensini积分分别为45.06±7.27、30.83±4.50、24.83±4.94和13.75±1.74,baPWV值分别为2.13±0.32、1.68±0.14、1.12±0.13和0.93±0.17 m/s,组间差异具有统计学意义(P<0.05)。冠心病组baPWV值与冠状动脉Gensini积分呈正相关。结论:baPWV值与冠状动脉粥样硬化的病变范围密切相关,可作为诊断冠心病和预测其病变严重程度的辅助指标。 Objective: To examine the relationship between brachial-ankle pulse wave velocity (baPWV) and the severity of coronary artery lesions. Methods: 198 patients who were going to be detected by coronary angiography (CAG) were selected, 148 patients were divided into coronary artery disease (CAD) group (according to the Lumen of blood vessel diameter stenosis -〉 50%) and 50 patients were divided into non-coronary artery disease group (control group). The severity of CAD was code as 1, 2 or more branches according to the number of major coronary vessels with stenosis, baPWV was measured using arteriosclerosis determinator VP-1000 and Gensini scoring system should be taken to evaluated the severity of coronary artery lesions in all patients within 24 hours after in hospital, then, the relationship between baPWV and coronary Gensini scoring was analyzed. Results: The Gensini scores of CAD group with more branches, 2 branches, 1 branch stenosis and control group were respectively 45.06_+ 7.27, 30.83_+ 4.50, 24.83_+ 4.94 and 13.75_+ 1.74, baPWV were 23.14+ 5.32, 19.72-+ 5.06, 16.98_+ 4.23 and 13.06_+ 4.08 m/s, progressively increased with the severity of CAD, baPWV of CAD group was obviously higher than that of control group (P〈0.05). There were significant positive correlation between baPWV and Gensini scoring in CAD group (P〈0.01). Conclusion: baPWV was closely related to the severity of coronary artery lesions, which could be used as an indicator of diagnosis and evaluation of severity of coronary artery lesions.
