2种材料复合用于犬上前牙拔牙位点保存的实验研究 被引量:8

Application of two kinds of composite materials in site preservation techniques in anterior extraction sockets in dogs

摘要 目的探讨富血小板纤维蛋白(platelet-rich fibrin,PRF)与羟基磷灰石生物陶瓷复合物用于犬上前牙拔牙位点即刻保存实验研究的可行性。方法 6只Beagle犬随机分为3组各2只,拔除上颌左右第二切牙后,A组植入PRF与羟基磷灰石生物陶瓷复合物,B组植入羟基磷灰石生物陶瓷,C组为空白对照组,分别于术后4、12周每组各处死1只犬,制备组织标本,通过大体观察、形态学测量以及锥形束CT检查评价拔牙窝愈合情况。结果 3组犬4、12周时唇舌侧牙槽嵴宽度差值比较差异均有统计学意义(P<0.05),A组差值最小;4周与12周时拔牙窝唇侧骨吸收程度A组阻止牙槽骨吸收效果最好,B组次之,C组最差;12周时A组牙槽嵴丰满度优于B组和C组(P<0.05);术后4周与12周CT值与术前CT值之差A组均高于B组和C组(P<0.05)。结论 PRF与羟基磷灰石生物陶瓷复合物是一种良好的新型复合骨移植材料,具有减缓牙槽嵴吸收,促进新骨形成,保存拔牙位点的作用。 Objective To discuss the feasibility of platelet-rich fibrin (PRF) and coralline hydroxyapatite composite in instant site preservation techniques in anterior extraction sockets. Methods Six healthy beagle dogs were randomly divided into 3 groups, with 2 dogs in each group. After the extraction of both maxillary incisors, group A was immediately implanted PRF/coralline hydroxyapatite composite, group B was implanted coralline hydroxyapatite, and group C was as blank control. One dog from each group was sacrificed 4 weeks later and the other dog was sacrificed 12 weeks later to prepare tissue samples. The healing of extraction socket was evaluated by gross observation, morphological measurements and photographs of cone beam CT. Results There were significant differences in the labial-lingual alveolar ridge width difference among these three groups both 4 and 12 weeks later (P^0.05), and the labial-lingual widtht difference was the smallest in group A. In 4 weeks and 12 weeks, the re-absorption in extraction area bone was the best in group A, followed by group B, and was the worst in group C. The fullness of alveolar ridge contour was better in group A than that in group 13 and group C in 12 weeks (P^0.05) . The difference between postoperative cone beam CT value and preoperative cone beam CT value was higher in group A than that in group B and group C in 4 and 12 weeks (P〈0.05). Conclusions PRF/coralline hydroxyapatite composite is a good new bone graft material, which can reduce the absorption of alveolar ridge, promote new bone formation, and preserve the socket site.
