摘要
目的比较复方苦参注射液联合顺铂及顺铂单药胸腔灌注后对恶性胸腔积液的近期疗效及不良反应。方法将2010年1月～2011年1月经新疆医科大学附属肿瘤医院细胞学确诊为恶性胸水的患者共62例随机分为两组,各31例,治疗组给予复方苦参注射液加顺铂胸腔注入,对照组单用顺铂胸腔注入。每周1次,连用4次后评价疗效。结果治疗组完全缓解(CR)14例,部分缓解(PR)10例,无效(NC)7例,有效率为77.4%。对照组CR 7例,PR 9例,NC 15例,有效率为51.6%,两组间有效率比较差异有统计学意义(P<0.05)。治疗组患者生活质量改善率明显高于对照组,两组比较,差异有统计学意义(χ2=5.599,P<0.05)。治疗组消化道反应(多为Ⅰ～Ⅱ度)发生率较对照组低(12.9%比35.5%),差异有统计学意义(P<0.05),两组中性粒细胞计数下降发生率比较(19.4%比45.1%),差异有统计学意义(P<0.05)。两组均无明显的肝肾功能损害及心脏毒性。结论在恶性胸腔积液治疗中,复方苦参联合顺铂治疗有效率高,不良反应少,且患者均可耐受,值得应用。
Objective To investigate clinical efficacy and adverse reactions of Compound Matrine Injection combined with Cisplatin and Cisplatin monotherapy thoracic perfusion in treatment of malignant pleural fluid.Methods 62 cases of patients with malignant pleural fluid diagnosed by cytology from January 2010 to January 2011 in Cancer Hospital Affiliated to Xinjiang Medical University were randomly divided into two groups,with 31 cases in each group,patients in treatment group were given Compound Matrine Injection and Cisplatin thoracic perfusion.Patients in control group were only given Cisplatin thoracic perfusion.All patients were given one time of treatment in one week,after 4 weeks treatment,the clinical efficacy were compared between two groups.Results In treatment group,there were 14 cases of CR,10 cases of PR and 7 cases of NC,effective rate was 77.4％; and in control group,there were 7 cases of CR,9 cases of PR and 15 cases of NC,the effective rate was 51.6％; the difference of effective rate between the two groups was statistically significant （P ＜ 0.05）.The life quality improvement rate of patients in treatment group was obviously higher than that of control group,the difference was statistically significant （x2=5.599,P ＜ 0.05）.The gastrointestinal reaction （mostly I-I degrees） incidence of treatment group was less than that of control group （12.9％ vs 35.5％）,the difference was statistically significant （P ＜ 0.05）,the neutrophils count decrease rate of two groups was compared （19.4％ vs 45.1％）,the difference was statistically significant （P ＜ 0.05）.No significant liver and kidney dysfunction and cardiac toxicity was found in two groups.Conclusion In the treatment of malignant pleural fluid,the effective rate of Compound Matrine Injection plus Cisplatin treatment is higher,adverse reaction is less,and patients can tolerate,so it is worthy of application.
