黄芪注射液对缺血缺氧性脑损伤新生儿免疫功能的影响 被引量:11

Effect of Astragalus membranaceus injection on immune function of neonates with hypoxic- ischemic brain damage

摘要 目的:探讨黄芪注射液对缺血缺氧性脑病(HIE)新生儿免疫功能的影响及意义。方法:选择HIE患儿60例随机分为黄芪注射液治疗组与常规治疗组各30例和同期出生正常新生儿30例作为对照组,采用ELISA和RT-PCR方法检测患儿血清中细胞因子(IL-2、IFN-γ和TNF-α)的蛋白及mRNA变化水平,采用流式细胞学检测患儿血清中T淋巴细胞亚群及免疫球蛋白的变化。结果:与对照组相比较,治疗前HIE患儿血清中TNF-α和CD8+均明显升高,差异有统计学意义(P<0.01),IL-2、IFN-γ、CD3+、CD4+、CD4+/CD8+、IgM均明显降低,差异有统计学意义(P<0.01),IgA、IgG差异无统计学意义(P>0.05)。与治疗前相比较,黄芪注射液治疗后HIE患儿血清中TNF-α和CD8+均明显降低,差异有统计学意义(P<0.01),IL-2、IFN-γ、CD3+、CD4+、CD4+/CD8+、IgM均明显升高,差异有统计学意义(P<0.01),IgA、IgG差异无统计学意义(P>0.05);基础常规治疗后HIE患儿血清中TNF-α和CD8+略降低,差异无统计学意义(P>0.05),IL-2、IFN-γ、CD3+、CD4+、CD4+/CD8+、IgM略升高,差异无统计学意义(P>0.05),IgA、IgG差异仍无统计学意义(P>0.05)。结论:黄芪注射液结合常规治疗能通过明显降低HIE患儿血清中TNF-α、CD8+含量和提高IL-2、IFN-γ、CD3+、CD4+、CD4+/CD8+、IgM含量而提高新生儿免疫力,为HIE患儿减轻免疫功能紊乱及继发感染提供理论依据。 Objective: To explore the effect of Astragalus membranaceus injection on immune function of neonates with hypoxic - ischemic encephalopathy (HIE) and its significance. Methods: Sixty neonates with HIE were selected and randomly divided into Astragalus membranaceus injection therapy group and conventional therapy group, 30 neonates in each group; 30 normal neonates born during the same period were selected as control group; the changes of levels of interleukin -2 (IL- 2), interferon -γ (IFN -γ), tumor necrosis factor- α (TNF -α) proteins and mRNA in serum of the neonates in the three groups were detected by ELISA and RT -PCR; flow cytometrY was used to detect the change of serum T lymphocyte subgroups and immunoglobulins. Results: Compared with control group, the levels of serum TNF - α and CD8+ before treatment in neonates with HIE increased significantly, there were statistically significant differences ( P 〈 0. 01 ), the levels of serum IL - 2, IFN - γ, CD3 + , CD4 + , CD4 +/CD8 + , and IgM decreased significantly, there were statistically significant differences (P 〈0. 01 ), there was no statistically significant difference in the levels of serum IgA and IgG between the two groups (P 〉 0. 05) . Compared with before treatment, the levels of serum TNF -α and CD8 + in Astragalus membranaceus injection therapy group de- creased significantly, there were statistically significant differences ( P 〈 0. 01 ), the levels of serum IL - 2, IFN - γ, CD3 + , CD4 + , CD4 +/CD8 + , and IgM increased significantly, there were statistically significant differences (P 〈 0. 01 ), there was no statistically signifi- cant difference in the levels of serum IgA and IgG between before and after treatment (P 〉 0. 05 ) ; after conventional therapy, the levels of serum TNF -α and CD8 + in neonates with HIE decreased slightly, there was no statistically significant difference ( P 〉 0. 05 ), the levels of serum IL- 2, IFN-γ, CD3 +, CD4 +, CD4 +/CD8 +, and IgM increased slightly, there was no statistically significant difference (P 〉 0. 05), there was no statistically significant difference in the levels of serum IgA and IgG between before and after treatment (P 〉 0. 05) . Conclusion: Astragalus membranaceus injection combined with conventional therapy can improve neonatal immunity by obviously reducing serum TNF - α and CD8 + contents and increasing serum IL - 2, IFN -γ, CD3 + , CD3+ , CD4+/CD8 + , and IgM contents, which provide a theoretical basis for relieving immune dysfunction and secondary infection among neonates with HIE.
