Effect of a water-cooled Nd:YAG laser in the non-surgical treatment of generalized aggresive periodontitis

摘要 目的:探讨neodymium:yttrium aluminium garnet(Nd:YAG)水激光在侵袭性牙周炎的非手术治疗中的辅助治疗作用。方法:对10名广泛型侵袭性牙周炎患者338个位点,采用单盲随机分口对照设计,对照侧采用超声龈上龈下洁刮治和手工器械根面平整(scaling and root planing,SRP),实验侧在SRP后用Nd:YAG水激光照射(SRP+Nd:YAG)。8周后比较各临床指标变化。结果:与基线水平相比,两侧各临床指标有显著改善(P<0.01)。SRP+Nd:YAG治疗侧在牙周探诊深度(pocket probing depths,PPDs)与单纯SRP治疗侧比较有显著差异(P<0.05),菌斑指数(Plaque index,PLI)和出血指数(bleeding index,BI)二者无显著差异(P>0.05)。结论:8周复查时,Nd:YAG水激光在侵袭性牙周炎的非手术治疗中的有一定的辅助治疗作用。 Objective :The aim of this study was to investigate whether use of a water-cooled neodymium :yttrium-aluminium-garnet (Nd:YAG) laser adjunctive to the non-surgical treatment of generalized aggresive periodontitis .Method :This study was performed according to an examiner-blind ,random split-mouth-design ,controlled clinical trial for sulcular debridement on 338 sites from 10 patients with generalized aggresive periodontitis .Immediately following scaling and root planing (SRP) in two randomly chosen contra -lateral quadrants ,test sides were additionally treated with the Nd :YAG laser (1064 nm ,6W , 400mJ) .Clinical assessments (Plaque index ,bleeding index ,pocket probing depths ) were performedpre-treatment and at 8 weeks post -treatment .Result :At the 8-week visit ,the clinical parameters had signi ?cantly improved for both regimens .There was postoperative improvement of all clinical parameters in both groups (P〈0 .01) ,and test side PPDs recovery was higher than that of the control side (p〈0 .05) ,but no signi?cant differences between treatment modalities were observed for PLI and BI .Conclusion :8 weeks after SRP ,partial additional advantage was achieved with the additional use of the Nd :YAG laser .
