哮喘急性加重和糖皮质激素治疗对诱导痰炎症细胞分类及表型的影响 被引量:18

Effects of steroid therapy and acute exacerbation of asthma on subtypes and pheno types in induced sputum inflammatory cells

摘要 目的研究糖皮质激素治疗及急性加重对哮喘患者诱导痰炎症细胞分类及表型的影响。方法收集2011年1-6月,新桥医院呼吸内科门诊和住院部经糖皮质激素治疗的104例哮喘患者的病史及诱导痰资料进行回顾性分析。结果急性加重患者诱导痰炎症细胞表型分别为中性粒细胞性42.3%,嗜酸性粒细胞性26.9%,混合细胞性及寡细胞性各为15.4%;非急性加重患者表型分别为嗜酸性粒细胞性50%,寡粒细胞性19.2%,混合细胞性19.2%,中性粒细胞性11.5%。急性加重及非急性加重患者痰炎症细胞分类均以中性粒细胞[(65.2±19.3)%、(54.4±20.5)%]及嗜酸性粒细胞[(3.8±7.4)%、(9.9±23.9)%]为主。急性加重组经糖皮质激素治疗前后诱导痰细胞炎症细胞表型及分类无显著变化(P>0.05),非急性加重组经吸入糖皮质激素治疗后诱导痰细胞中性粒细胞比例从(55.5±20.5)%上升到(68.9±19.3)%,嗜酸性粒细胞从(14.7±20.1)%下降为(4.6±5.6)%,两者呈显著的负相关性(r=-0.66,P<0.05);中性粒细胞性表型患者的比例也显著增多(P<0.05)。结论哮喘急性加重及吸入激素治疗均可以导致哮喘患者诱导痰中性粒细胞增多,改变气道炎症细胞的表型。 Objective To determine the effects of steroid therapy and acute exacerbation of asthma on the subtypes and phenotypes in induced sputum inflammatory cells from asthma patients.Methods Clinical data on medical history and induced sputum of 104 asthma out-and in-patients from our institute during January 2011 to June 2011 were collected and retrospectively analyzed.Results There were 42.3% of acute patients found with neutrophilic phenotype,26.9% with eosinophilic phenotype,15.4% with paucigranulocytic phenotype,and 15.4% with mixed granulocytic subtypes.In non-acute asthma patients group,50.0% patients had the eosinophilic phenotype,19.2% had paucigranulocytic phenotype,19.2% had mixed granulocytic subtypes,and 11.5% had neutrophilic phenotype.The major subtypes of sputum inflammatory cells were neutrophilic [(65.2 ± 19.3) % and(54.4 ± 20.5) %] and eosinophilic phenotypes [(3.8 ± 7.4) % and(9.9 ± 23.9) %]in acute and non-acute asthma patients,respectively.There was no obvious change in the subtypes and phenotypes of induced sputum inflammatory cells from acute asthma group before and after steroid treatment.After treatment with glucocorticoid in non-acute asthma group,neutrophil percentage was raised from(55.5 ±20.5) % to(68.9 ± 19.3) %,and proportion of eosinophilic phenotype was reduced from(14.7 ± 20.1) % to(4.6 ± 5.6) %.There existed significant negative correlation between them(r =-0.66,P 0.05).Proportion of neutrophilic phenotype was also raised(P 0.05).Conclusion Both of acute exacerbation of asthma and inhaled corticosteroid therapy impact asthma inflammatory phenotypes and increase neutrophils in induced sputum.
