摘要
目的了解2011年1月-2012年12月临床分离病原菌的分布和耐药情况,为指导临床合理用药和医院感染的控制提供依据。方法采用法国梅里埃API半自动微生物鉴定分析系统进行细菌鉴定和药物敏感试验,对所得数据用WHONET 5.4软件完成统计分析。结果送检的3 073份临床标本中共分离出病原菌696株,阳性率22.1%。其中,G+菌占31.3%,G菌占64.1%,主要为金黄色葡萄球菌(25.6%)、大肠埃希菌(17.8%)、肺炎克雷伯菌(16.4%)、铜绿假单胞菌(11.5%)。在葡萄球菌中,青霉素、红霉素的耐药率最高,分别是95.6%、78.5%;尚无耐万古霉素菌株。肠道杆菌中,阿莫西林的耐药率最高,为95.0%;替卡西林的耐药率为91.0%;头孢噻吩的耐药率为72.0%;尚无耐亚胺培南菌株。多重耐药的大肠埃希菌、肺炎克雷伯菌对亚胺培南具有良好的敏感性。结论该院临床分离的致病菌耐药情况不严重,对病原菌的分布和耐药情况的研究较好地指导了临床合理用药和控制了医院感染。
Objective To get clear information about the clinical distribution of pathogenic bacteria and the drug resistance condition in our hospital from 2011 to 2012 so as to provide foundational information to instruct rational drug use.Methods The French API semi-auto microbial identifying and analyzing system was used to perform the bacteria identification and drug sensitiveness test and the data were analyzed with the software WHONET 5.4.Results A total of 696 strains of pathogenic bacteria were isolated from 3073 clinical specimens.The positive rate was 22.65%.The Gram-positive cocci shared 31.9% and the Gram-negative bacilli shared 63.7% of all these,and they mainly were Staphylococcus aureus(10.0%),Escherichia coli(10.0%),Klebsiella pneumonia(8.0%) and Pseudomonas aeruginosa(9.0%).The resistance rates of penicillin and erythrocin were respectively 95.6% and 78.5%,which were the top two in the staphylococci.The highest resistance rate in enteric bacilli was amoxicillin(95%),followed by ticarcillin(91%) and cephalothin(72%).No strain was found to be able to be resistant to Imipenem.Multidrug resistant Escherichia coli and Klebsiella pneumonia were quite sensitive to Imipenem.Conclusions The drug resistance of pathogenic bacteria in clinics in our hospital is not serious.This study on pathogenic bacteria distribution and drug resistance has instructed rational drug use in clinics and has controlled hospital infection well.
