摘要
目的探讨颅内后循环动脉瘤开颅手术与栓塞治疗后的效果和对脑组织损伤的影响。方法回顾性纳入2011年10月—2012年3月于北京大学第一医院神经外科诊治的27例颅内后循环动脉瘤患者,按治疗方式分为手术治疗组12例和栓塞治疗组15例,将两组患者的治疗情况、治疗后的格拉斯哥预后评分(GOS)、并发症发生率、复发率及治疗前后的血清S100B蛋白、基质金属蛋白酶9(MMP-9)、超氧化物歧化酶(SOD)水平进行比较。结果①栓塞治疗组手术时间及住院时间分别为(90±14)min和(14±4)d,手术治疗组分别为(129±21)min和(18±4)d,两组比较差异有统计学意义(P<0.05)。②栓塞治疗组治疗后的GOS评分恢复良好率为93.3%,手术治疗组为50.0%。③治疗后3 d和7 d的血清S100B蛋白、MMP-9水平栓塞治疗组均低于手术治疗组,而SOD水平则高于手术治疗组,两组比较差异均有统计学意义(均P<0.05)。结论颅内后循环动脉瘤栓塞治疗的临床效果更佳,并且对脑组织的侵袭程度也更小,利于患者的康复。
Objective To investigate the effects of surgical and embolization treatment of intracranial posterior circulation aneurysms and their impact on brain damage associated factors during the treatment. Methods A total of 27 patients with intracranial posterior circulation aneurysms were diagnosed and trea-ted in the Department of Neurosurgery, Peking University First Hospital from October 20ll to March 2012 were enrolled retrospectively. They were divided into either an surgical group (group A; n = 12) or a em- bolization group (group B; n = 15) according to their treatment modalities. The Glasgow Outcome Scale (GOS) scores after treatment, incidence of complications, recurrence rate, as well as the levels of serum SIOOB protein, MMP-9 and SOD before and after treatment were compared. Results ①The operation and hospitalization time and of group B were 89.6 ± 14.0 min and 14.1 ± 3.6 d respectively, and they were all shorter than 129.3 ± 21.4 rain and 18.2 ± 4.0 d of group A. The differences were statistically significant between the two groups (P 〈 0.05). ②The good recovery rate of the GOS score of group B after treatment was 93.3% , and it was higher than 50.0% of group A. The serum S100B protein and MMP at day 3 and 7 were lower than those of group A, while the SOD was higher than that of group A. The differences were statistically significant between the two groups (all P 〈 0.05). Conclusion The clinical result of embo- fization of posterior circulation intracranial aneurysms is better, and the degree of brain tissue invasion is al-so minor. It is conducive to the rehabilitation of patients.
