摘要
2011年北京市某医院在全国率先进行按病种组合预定额支付结算(DRGs-PPS)。为迎接这一挑战,该院长期以来以提高医院运行效率为发展战略,以缩短平均住院日为指标,进行目标管理。评价方法:采用描述性统计方法,统计该医院按DRGs结算单位权重费用,以及1996年至2012年的出院人次、手术人次、病例组合指数、次均费用。结果:该院在结算方式试点中医疗机构获得了较好的收益;其原因与平均住院日相关。结论:DRGs-PPS有激励医疗机构管理者提高效率作用;政府应推动这一改革,创造医疗机构竞争环境;医疗机构采用主诊医师的组织模式,引进内部竞争机制,进一步调动医生的积极性,使医生主动引入国际一流水平的临床路径,就可使我们的服务效率达到世界一流水平。
October 8, 2011 in a hospital in Beijing, Beijing launched the first DRGs-PPS (Diagnosis Related GroupsProspective Payment System) pilot in China. Conclusion: DRGs-PPS motivate managers of medical institutions to improve the efficiency of the role; Governments should promote this reform to create the competitive environment of medical institutions; medical institutions to adopt the organizational model of the attending physician, the introduction of internal competition mechanism, to further mobilize the enthusiasm of doctors, doctors take the initiative to introduce world-class clinical pathway, you can make the efficiency of our services to world-class level.
