摘要
目的 分析研究某院骨科平均住院日延长的影响因素.方法 基于DRGs指标,根据某院2018年1月-9月住院病案首页信息,经云南省医院质量管理及绩效平台CN-DRGs分组器的分组结果 ,筛选1388份骨科出院病例,选取进入同一DRG组,同为医嘱离院,住院天数相差1倍及以上,相似度高可比性强的病例进行对比分析.结果经统计分析骨科住院天数超额定平均住院日为10.5天,总计665例.影响骨科住院天数延长的病例主要集中在11天~18天段和大于34天段,分别占超额病例58.19%、6.02%;其中患者年龄大基础病多、术后并发症、入院检查不完善占比大,分别为40.75%、22.25%、15.69%.结论 缩短骨科平均住院日重点在临床诊疗过程,应建立健全感染管理制度,控制医院感染;利用临床路径进行病种管理,完善入院检查,缩短术前等候时间;加大平均住院日宣传力度,提高骨科工作效率.
Objectives The influence factors of the extension of the average hospital stay in a hospital were analyzed and studied.Methods Based on the DRGs indicator,according to the hospital's 2018 January-September Inpatient medical records home information by Yunnan Province Hospital Quality management and performance platform CN-DRGs grouping results,screening of 1388 cases of orthopaedics department discharge,select into the same DRG group,the same as the doctor's order to leave the hospital,the number of days of hospitalization is one times different and above,the similarity of high comparability of cases for comparative analysis.Results After statistical analysis,the average number of hospital days(10.5 days)in orthopaedics hospital was exceeded by 665 cases.The cases affecting the prolongation of the number of hospitalizations in orthopaedics hospital were mainly in the 11-18-day period and greater than 34 days,respectively,accounting for 58.19%and 6.02%of the excess cases.Among them,the patients had more age-based diseases,postoperative complications and imperfect admission examination,which were 40.75%,22.25%and 15.69%,respectively.Conclusions To shorten the average inpatient day in orthopaedics department was focused on the clinical diagnosis and treatment process,and a sound infection management system should be established to control nosocomial infection;The clinical pathway was used to manage the disease species,improve the admission examination and shorten the waiting time before operation;Increase the average inpatient day publicity efforts,improve the work efficiency of orthopaedics department.
