摘要
目的:探查机构内失能老人的照护需要及影响因素,分析需要满足情况。方法:采用简单随机抽样方法,选取南京市22家养老机构,对机构内388位失能老人进行了问卷调查。结果:①机构内失能老人健康状况不佳,平均患有1.96种慢性疾病。中度失能老人占近一半,ADL均值为13.0分。②失能老人最希望获得的服务依次是日常生活照料、保健护理、精神慰藉。PSMS、IADL是影响服务需要的主要因素。文化程度、患病情况、子女数量、选择机构的理由也影响着需要表达的强弱。③机构提供的服务总体上能够回应老人所需,但在部分项目上存在着供需错位问题。老人最满意的是住宿条件,最不满意的是精神慰藉,有五成老人对伙食不满意。结论:机构照护供给数量尚可,但照护质量亟待提高。建议拓展服务项目,确立"以需要为本"的运营策略。
Objective To explore the care needs and influencing factors and to analyze the situation of needs-satisfaction a-mong the disabled elderly in the pension institutions. Methods By using simple random sampling,22 pension institutions ,388 disa- bled olders in Nanjing were selected in a survey. Results ① The disabled elderly health status was poor, withl. 96 kinds of chron- ic diseases averagely;moderate disabled elderly accounted for nearly half; the mean value of ADL was 13 points. ②The disabled elderly most wanted to acquire the services of daily life care, health care, spiritual consolation. PSMS and IADL were the main in- fluencing factors. Cultural level, prevalence, the number of children and the reason for choose the pension institution also influ- enced the strength of expressing needs. ③Services of the pension institutions on the whole can respond to the needs of the disa- bled elderly ,but in some projects there were problems about dislocation of supply and demand. The most satisfactory service for disabled old men was the accommodation condition, the least satisfactory was the spiritual consolation, and Fifty percent of the dis- abled old men were not satisfied with the food. Conclusion Quantity supplied of institutional care is acceptable, but the quality must be improved promptly. So the pension institutions should expand service projects, establish operation strategy of taking need as foundation.
