摘要
目的研究吲哚菁绿介导的光栓疗法(indocyanine green mediated photothrombosis,i-MP)对年龄相关性黄斑变性(age-related macular degeneration,AMD)患者的最佳矫正视力(best corrected visual acuity,BCVA)及黄斑中心凹厚度(cretinal macular thickness,RMT)的影响。方法 11例(12眼)经眼底荧光血管造影(fluorescence fundus angiography,FFA)、吲哚菁绿血管造影(indocyanine green angiography,ICGA)确诊为2型脉络膜新生血管(choroidal neovascularization,CNV)的AMD患者,给予i-MP治疗。每次治疗后12周、48周,根据ICGA及FFA检查结果,决定是否行再次治疗(不超过3次)。第1次治疗结束后1周、12周、48周随访患者BCVA,同时检测RMT及黄斑形态。结果治疗前最佳矫正视力为1.12~1.53logMAR;治疗后1周,1眼视力提高,11眼视力稳定,未见视力下降患者;治疗后12周,4眼视力提高,6眼视力稳定,2眼视力下降;治疗后48周,5眼视力提高,5眼视力稳定,2眼视力下降。治疗前RMT为(391.34±69.89)μm,治疗后12周、48周分别为(337.73±56.23)μm、(332.14±62.07)μm,与治疗前比较差异均有统计学意义(均为P<0.05)。OCT证实83.3%的患者RMT有所下降,且黄斑形态改善。治疗后48周ICGA显示:CNV荧光素渗漏消失7眼,明显减轻3眼,加重或无明显好转2眼。治疗中及治疗后无明显并发症发生。结论 i-MP是一种治疗AMD黄斑下CNV的有效方法。
Objective To study the effects of indocyanine green-mediated photothrom-bosis( i-MP) on the best corrected visual acuity ( BCVA) and retinal macular thickness ( RMT) of patients with age-related macular degeneration( AMD) .Methods Eleven cases ( 12 eyes) were given i-MP treatment after being diagnosed as AMD which caused type Ⅱ choroidal neovascularization ( CNV) by fluorescence fundus angiography ( FFA) and indocyanine green angiography ( ICGA) .Operation of the next treatment depended on the results of FFA and IGCA was conducted at 12 weeks and 48 weeks after every treatment ( less than three times) .BCVA and RMT were examined at 1 week,12 weeks and 48 weeks after the first treatment.Results BCVA was from 1.12 to 1.53 before treatment; At 1 week after treatment,1 eye improved,11 eyes unchanged; At 12 weeks after treatment,4 eyes improved,6 eyes unchanged,while 2 eyes declined; At 48 weeks after treatment,5 eyes improved,5 eyes unchanged,while 5 eyes declined.RMT before treatment and 12 weeks,48 weeks after treatment were ( 391.34±69.89) μm,( 337.73±56.23) μm and ( 332.14±62.07) μm,respectively,the differences were statistically significant ( all P 0.05) .OCT affirmed that decrease of RMT and improvement of macular morphous were present in 83.3% of the cases.ICGA showed that fluorescein leakage was absent from the previous classic component of CNV in 7 eyes,significant decrease of CNV leakage in 3 eyes,increase or no significant decrease of CNV leakage in 2 eyes.No complications were observed during and after the treatment.Conclusion I-MP may be an effective treatment for subfoveal CNV caused by AMD.
