摘要
目的:检测心力衰竭患者血清脑钠肽(BNP)及N末端脑钠肽前体(NT-proBNP)水平,分析其与心功能的相关性以及对心力衰竭的诊断价值。方法:选取我科治疗的慢性心衰患者58例并按照NYHA心功能分级,比较不同心功能分级的患者血清BNP、NT-proBNP的水平及其与射血分数等指标的相关性,并分析BNP与NT-proBNP的诊断价值。结果:①各心功能分级间采用单因素方差分析发现,BNP、NT-proBNP随心功能分级增加而增加(P<0.05),不同心功能分级的组间比较发现,除III级与IV级BNP、NT-proBNP、LVEF、6min步行距离差异不明显(P>0.05),其余组间比较均具有显著性差异(P<0.05)。②经比较发现,BNP、NT-proBNP均与LVEF、6min步行距离呈负相关,而与左室舒张末期内径、左室收缩末期内径呈正相关。③NT-proBNP诊断效力高于BNP。结论:血清BNP和NT-proBNP可作为评估左心室功能不全和左心室结构改变的临床指标。
To study the levels of serum BNP (Brain natriuretic peptide, BNP) and NT-proBNP (N-terminal pro-brain natriuretic peptide, NT-proBNP) in patients with CHF (chronic heart failure, CHF), to investigate their diagnostic values and analyze their relativity with different NYHA heart function. Methods: Totally, 58 patients with CHF were selected. Serum BNP and NT-proBNP were measured with automatic enzyme immunoassay system and chemiluminescence immunoassay respectively. Comparing the relation- ship between BNP and NT-proNBP in different stages of heart function and analysis their diagnosis value. Results: ①The level of BNP, NT-proBNP were significantly increased with the stages of heart function grow up (P〈0.05). But the difference of BNP, NT-proBNP LVEF and 6min walk test between NYHA III and NYHA IV were not obviously (P〈0.05) ②The level ofBNP, NT-proBNP were nega-tively correlated with LVEF and 6min walk test and were positively correlated with LVEDD and LVESD. ③ The diagnostic value of NT-proBNP is higher than BNP. Conclusion: Both plasma BNP and NT-proBNP can be used to evaluate the heart function of patients with heart failure, and are useful tools for diagnosis of CHF.
